摘要
目的 分析难治性视网膜静脉阻塞继发黄斑水肿的血流动力学特征,以期为临床诊治提供参考.方法 描述性病例研究.纳入2019年1月至2021年12月在安徽医科大学附属巢湖医院眼科确诊的难治性视网膜静脉阻塞继发黄斑水肿的46例(46只眼)患者作为病例组,其中男性26例(26只眼),女性20例(20只眼),年龄(63.6±1.3)岁,对照组选取40例(40眼)非难治性视网膜静脉阻塞继发黄斑水肿的患者,男性17例,女性23例,年龄(64.9±1.5)岁,收集两组患者的平均动脉压、心率、红细胞、血小板,凝血酶时间,并记录视网膜静脉收缩期峰值流速(peak systolic velocity,PSV)、舒张末期流速(end diastolic velocity,EDV)、阻力指数(esistance index,RI)、及臂视网膜时间(arm-retinal time,ART),眼压值.结果 46例难治性RVO继发黄斑水肿的患者中,合并高血压25例(54.3%),吸烟12例(26.1%),平均动脉压(93,112.3)mmHg、心率(76.5±7.7)次/分、红细胞(4.02,4.66)× 109/L、血小板(174.5±52.0)×109/L、凝血酶时间(18.0±1.3)s、PDV(7.5±1.8)cm/s、EDV(2.1,2.7)cm/s、RI(0.68±0.01)、ART(16.1±0.4)s.与对照组相比,患者平均动脉压改变有统计学意义(z=-2.754,P=0.006),而红细胞、血小板、心率、PDV、EDV及凝血酶时间改变均无统计学意义(P>0.05),且合并基础疾病均无统计学意义,结论 难治性RV0继发黄斑水肿可能与平均动脉压相关.
Abstract
Objective To analyze the hemodynamic characteristics of macular edema secondary to refractory retinal vein occlusion in order to provide reference for clinical diagnosis and treatment.Methods Descriptive case study.46 patients(46 eyes)were included as the case group,with macular edema secondary to refractory retinal vein occlusion in the ophthalmology department of Chaohu Hospital of Anhui Medical University from January 2019 to December 2021,including 26 males(26 eyes)and 20 females(20 eyes),average age(63.6±1.3)years.40 patients(40 eyes)with macular edema secondary to refractory retinal vein occlusion were selected as the control group,including 17 males and 23 females,The mean arterial pressure,heart rate,red blood cells,platelets,and thrombin time of the patients in the two groups were collected,and the peak systolic velocity(PSV),end diastolic velocity(EDV),resistance index(RI),arm retinal time(ART)and intraocular pressure were recorded.Results Among 46 patients with macular edema secondary to refractory RVO,25 patients(54.3%)had hypertension,12 patients(26.1%)had smoking,mean arterial pressure(93,112.3)mmHg,heart rate(76.5±7.7)times/minute,and red blood cells(4.02,4.66)× 109/L,platelet(174.5±52.0)× 109/L,thrombin time(18.0±1.3)s,PDV(7.5±1.8)cm/s,EDV(2.1,2.7)cm/s,RI(0.68±0.01),ART(16.1±0.4)s.Compared with the control group,the change of mean arterial pressure in patients was statistically significant(z=-2.754,P=0.006),while the changes of red blood cells,platelets,heart rate,PDV,EDV and thrombin time were not statistically significant(P>0.05),and there was no statistical significance in patients with underlying diseases.Conclusion The macular edema secondary to refractory RVO may be related to mean arterial pressure.
