异嗜性抗体对甲状腺功能指标检测的干扰作用

摘要
目的 分析异嗜性抗体(HAb)对甲状腺功能指标检测的干扰作用.方法 选择2020年7月—2022年7月济南市中西医结合医院收治的280例血浆促甲状腺激素(TSH)水平不当递增的患者作为研究对象,其中20例为疑似HAb增多症.采用放射免疫性鉴定法(RIA)和放射免疫分析模式(IRMA)检测TSH,排除血清类风湿因子等干扰后,检测血清标本的HAb水平;采用固相放免分析检测两组的游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4).结果 采用RIA和IRMA方法检测疑似HAb增多症患者的TSH水平均明显低于其他TSH不适当递增患者[mU/L:RIA法5.52±0.65比16.08±3.10;IRMA法4.02±0.42比 16.03±2.96;均P<0.05].疑似HAb增多症患者的FT3、FT4 水平与其他TSH不当递增患者比较差异均无统计学意义[FT3(ng/L):3.25±0.85 比 3.21±0.66;FT4(ng/L):1.38±0.22 比 1.14±0.10;均P>0.05].结论 HAb可能影响甲状腺功能指标检测结果,需根据实际情况进行分析,尽量减少或排除该影响.
Abstract
Objective To analyze the interfering effect of heterophilic antibody(HAb)on detection of thyroid function indexes.Methods A total of 280 patients with inappropriate increased level of plasma thyroid stimulating hormone(TSH)in Jinan Hospital of Integrated Chinese and Western Medicine from July 2020 to July 2022 were selected as research objects.Among them,20 patients were suspected with HAb hyperplasia.TSH was detected using radioimmunoassay(RIA)and immunoradiography(IRMA).After excluding interference from serum rheumatoid factors,the level of HAb in serum samples was measured.The levels of free triiodothyronine(FT3)and free tetraiodothyronine(FT4)were detected using solid-phase radioimmunoassay.Results Using RIA and IRMA methods,the TSH level in patients with suspected HAb hyperintensities was lower than that in patients with improper TSH increase[mU/L:RIA 5.52±0.65 vs.16.08±3.10;IRMA 4.02±0.42 vs.16.03±2.96;both P<0.05].There was no significant difference in FT3 and FT4 levels between patients with suspected HAb hyperplasia and other patients with improper TSH increase[FT3(ng/L):3.25±0.85 vs.3.21±0.66;FT4(ng/L):1.38±0.22 vs.1.38±0.22;both P>0.05].Conclusion HAb might affect the detection results of thyroid function indicators,and analysis should be conducted based on actual conditions to reduce or exclude the impact.
