肝素抗凝动脉血全血与血清在临床生化检验中的应用效果

摘要
目的 研究肝素抗凝动脉血全血与血清在临床生化检验中的应用效果.方法 选择 2020 年7 月—2022 年 7 月在聊城市第二人民医院进行体检的 88 例健康体检者作为研究对象.采集受检者血清以及肝素抗凝动脉血全血,分别应用全自动生化分析仪和血气分析检测法测定所有受检对象葡萄糖(Glu)、钾离子(K+)、钠离子(Na+)、氯离子(Cl-),并对结果进行比较.结果 肝素抗凝动脉血全血组和血清组的Cl-水平比较差异无统计学意义(mmol/L:101.70±7.05 比 102.74±4.94,P>0.05),肝素抗凝动脉血全血组的Glu明显高于血清组检测值(mmol/L:8.41±4.06 比 6.13±2.43,P<0.05),肝素抗凝动脉血全血组的K+、Na+水平均明显低于血清组检测值,差异均有统计学意义[K+(mmol/L):3.67±0.64 比 3.95±0.50,Na+(mmol/L):132.74±5.36 比 138.03±4.24,均P<0.05];肝素抗凝动脉血全血组的检测准确率明显高于血清组[98.86%(87/88)比 92.05%(81/88),P<0.05].结论 肝素抗凝动脉血全血的临床生化检验结果准确率较血清标本更高,但在临床实际应用过程中,不能简单用肝素抗凝动脉血全血代替血清进行检测.
Abstract
Objective To investigate the application effect of heparin anticoagulation arterial whole blood and serum in clinical biochemical test.Methods A total of 88 health examiners from July 2020 to July 2022 in the Second People's Hospital of Liaocheng City were selected as research objects.The serum and heparin anticoagulation arterial whole blood were collected,and the levels of blood glucose(Glu),blood potassium(K+),blood sodium(Na+)and blood chloride(Cl-)were detected by automatic biochemical analyzer and blood gas analysis method,respectively,and the results were compared.Results There was no significant difference in the level of Cl-between heparin anticoagulation arterial whole blood group and serum group(mmol/L:101.70±7.05 vs.102.74±4.94,P>0.05).The level of Glu in heparin anticoagulation arterial whole blood group was significantly higher than that in serum group(mmol/L:8.41±4.06 vs.6.13±2.43,P<0.05),and the levels of K+ and Na+ in heparin anticoagulation arterial whole blood group were significantly lower than those in serum group,the differences were statistically significant[K+(mmol/L):3.67±0.64 vs.3.95±0.50,Na+(mmol/L):132.74±5.36 vs.138.03±4.24,both P<0.05].The detection accuracy of heparin anticoagulant arterial whole blood group was significantly higher than that in serum group[98.86%(87/88)vs.92.05%(81/88),P<0.05].Conclusions Compared with the serum samples,the clinical biochemical test results using heparin anticoagulation arterial whole blood are more accurate.However,in the process of clinical application,it is not possible to simply substitute heparin anticoagulant arterial whole blood for serum.
