摘要

脓毒症是急诊科及重症监护病房常见的感染性疾病之一,通常有不同感染途径,如呼吸道、泌尿道等.脓毒症通常直接或间接引起不同器官功能损伤,急性肾损伤(AKI)是脓毒症最常见的并发症之一.通常临床识别肾损伤是根据患者的尿量及肌酐(Cr)水平的变化,然而通过尿量与Cr来识别肾损伤不仅有滞后效应,并且尿量减少与Cr水平上升并不参与肾损伤的发生过程.因此通过了解脓毒症导致肾损伤的机制来找到能早期、准确识别肾损伤发生的生物标志物或其他诊断方法显得尤为重要.该文就脓毒症相关急性肾损伤(SAKI)的发生机制及早期诊断方法进行综述.

Abstract

Sepsis is one of the common infectious diseases in emergency department and intensive care unit.It usually has different infection routes,such as respiratory tract,urinary tract and so on.Sepsis usually directly or indirectly causes different organ function damage,acute kidney injury(AKI)is one of the most common complications of sepsis.Usually,kidney injury was identified according to the patient's urine output and creatinine(Cr)level.However,the identification of kidney injury by urine output and Cr level not only has a hysteresis effect,but also the decrease of urine output and the increase of Cr level do not participate in the occurrence of kidney injury.Therefore,it is particularly important to find biomarkers or other diagnostic methods that could accurately identify the occurrence of kidney injury by understanding the mechanism of sepsis.This article reviews the mechanisms and early diagnosis methods of sepsis related acute kidney injury(SAKI).
