摘要
目的 研究生化检验在糖尿病诊断中的应用效果.方法 选择2021年9月—2023年9月在单县园艺社区卫生服务中心就诊的 60 例糖尿病患者与同时间段 60 例健康体检者作为研究对象,分别纳入糖尿病组和健康对照组.对所有受检者进行生化检验,应用氧化酶法检测空腹血糖(FBG)和餐后 2 h血糖(2hPG),应用糖化血红蛋白分析仪测定糖化血红蛋白(HbA1c),应用全自动生化分析仪测定总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C);比较HbA1c、TG单独与联合检测的检出率.结果 糖尿病组的FBG、HbA1c、2hPG、TC、TG、LDL-C水平均显著高于健康对照组,HDL-C水平显著低于健康对照组[FBG(mmol/L):9.18±2.01比5.31±0.47;HbA1c:0.073±0.008比0.051±0.004;2hPG(mmol/L):12.03±2.46 比 6.10±0.62;TC(mmol/L):5.89±0.63 比 3.69±0.50;TG(mmol/L):2.95±0.47比 1.20±0.22;LDL-C(mmol/L):3.68±0.84 比 2.08±0.42;HDL-C(mmol/L):0.88±0.08 比 1.45±0.16;均P<0.05].HbA1c和TG联合检测的检出率明显高于两项指标单独检测[98.33%(59/60)比83.33%(50/60)、63.33%(38/60),均P<0.05].结论 生化检验中监测血糖和血脂指标的变化有助于临床诊断糖尿病,多指标联合检测可提高疾病的检出率.
Abstract
Objective To investigate the effect of biochemical tests in the diagnosis of diabetes mellitus.Methods The 60 patients with diabetes mellitus who visited Shanxian County Horticultural Community Health Service Center from September 2021 to September 2023 and 60 healthy examinees in the same period were selected as research objects,and were respectively included in diabetes mellitus group and healthy control group.The patients in both groups underwent biochemical tests,the levels of fasting blood glucose(FBG)and 2-hour postprandial blood glucose(2hPG)were detected using oxidase method,the level of glycosylated hemoglobin(HbA1c)was detected using HbA1c analyzer,and the levels of total cholesterol(TC),triacylglycerol(TG),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)were measured using fully automated biochemical analyzer.The detectable rates of HbA1c and TG separately and in combination were compared.Results The levels of FBG,HbA1c,2hPG,TC,TG and LDL-C in diabetes mellitus group were significantly higher than those in healthy control group,while the level of HDL-C was significantly lower than that in healthy control group[FBG(mmol/L):9.18±2.01 vs.5.31±0.47;HbA1c:0.073±0.008 vs.0.051±0.004;2hPG(mmol/L):12.03±2.46 vs.6.10±0.62;TC(mmol/L):5.89±0.63 vs.3.69±0.50;TG(mmol/L):2.95±0.47 vs.1.20±0.22;LDL-C(mmol/L):3.68±0.84 vs.2.08±0.42;HDL-C(mmol/L):0.88±0.08 vs.1.45±0.16;all P<0.05].The detectable rate of HbA1c and TG combined detection was significantly higher than those of HbA1c and TG detected alone[98.33%(59/60)vs.83.33%(50/60),63.33%(38/60),both P<0.05].Conclusion Monitoring the changes of blood glucose and blood lipid indicators in biochemical tests is helpful for clinical diagnosis of diabetes mellitus,and joint detection of multiple indicators could improve the detectable rate of disease.
