摘要

目的 研究妊娠期糖尿病(GDM)与孕妇糖脂代谢指标的相关性.方法 选择2020年10月—2023 年 10 月三明市第二医院收治的 159 例GDM孕妇纳入GDM组,另外选择同期于该院进行产前检查的159 例健康孕妇作为对照组.使用全自动血液分析仪,采用流式细胞术检测血红蛋白(Hb);使用全自动生化分析仪检测血脂指标,采用直接法检测高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C),采用氧化酶法检测总胆固醇(TC)、三酰甘油(TG);使用全自动糖化血红蛋白(HbA1c)测定仪,采用高效液相色谱法检测HbA1c水平.比较两组上述指标水平差异;应用Spearman相关性分析方法考察各指标与GDM的相关性.结果 GDM组Hb和HDL-C水平均显著低于对照组,TC、TG、LDL-C、HbA1C水平均显著高于对照组,差异均有统计学意义[Hb(g/L):60.00±8.62 比 142.02±8.50;HDL-C(mmol/L):1.66±0.03 比1.80±0.05;TC(mmol/L):6.27±0.21 比 3.95±0.10;TG(mmol/L):3.06±0.10 比 2.20±0.05;LDL-C(mmol/L):2.87±0.05 比 2.25±0.02;HbA1c:0.062±0.006 比 0.053±0.005;均P<0.05].TC、TG、LDL-C与GDM均呈中度正相关(r值分别为 0.397、0.290、0.509,P值分别为 0.001、0.014、<0.001).HDL-C、Hb与GDM均呈中度负相关(r值分别为-0.394、-0.294,P值分别为0.001、0.013).结论 HbA1c、TC、LDL-C水平升高以及HDL-C和Hb水平降低均为GDM的促成因素.怀孕期间监测和管理Hb、血脂指标和HbA1c水平对预防和管理GDM具有重要意义,需要进一步研究GDM的潜在机制和干预措施.

Abstract

Objective To investigate the correlation between gestational diabetes mellitus(GDM)and glycolipid metabolism indicators in pregnant women.Methods A total of 159 pregnant women admitted to Sanming Second Hospital from October 2020 to October 2023 were selected as research subjects and included in GDM group,in addition 159 healthy pregnant women who underwent prenatal examination in the hospital during the same period were selected as control group.The level of hemoglobin(Hb)was detected using a fully automated blood analyzer and flow cytometry.The levels of blood lipid indicators were detected using a fully automated biochemical analyzer.The levels of high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)were detected using direct method,and the levels of total cholesterol(TC)and triacylglycerol(TG)were detected using oxidase method.The level of glycosylated hemoglobin(HbA1c)was detected using a fully automated HbA1c analyzer and high-performance liquid chromatography.The differences of above indicators between two groups were compared.Spearman correlation analysis was applied to explore the correlation between different indicators and GDM.Results The levels of Hb and HDL-C in GDM group were lower than those in control group,while the levels of TC,TG,LDL-C and HbA1C were higher than those in control group,with statistically significant differences[Hb(g/L):60.00±8.62 vs.142.02±8.50;HDL-C(mmol/L):1.66±0.03 vs.1.80±0.05;TC(mmol/L):6.27±0.21 vs.3.95±0.10;TG(mmol/L):3.06±0.10 vs.2.20±0.05;LDL-C(mmol/L):2.87±0.05 vs.2.25±0.02;HbA1c:0.062±0.006 vs.0.053±0.005;all P<0.05].TC,TG and LDL-C were moderately positively correlated with GDM(r values were 0.397,0.290 and 0.509,respectively,and P values were 0.001,0.014 and<0.001,respectively).HDL-C and Hb were moderately negatively correlated with GDM(r values were-0.394 and-0.294,respectively,and P values were 0.001 and 0.013,respectively).Conclusions The elevated levels of HbA1c,TC,LDL-C,as well as the decreased levels of HDL-C and Hb,are considered to be contributing factors to GDM.Monitoring and managing the levels of Hb,blood lipid indicators and HbA1c during pregnancy is of great significance for prevention of GDM,and further research is needed on the potential mechanisms and intervention measures of GDM.
