Objective To analyze the application and accuracy of blood routine test in patients with iron deficiency anemia(IDA)and thalassemia(TM).Methods A total of 100 patients with anemia admitted to Shanghang County Hospital from February 2023 to January 2024 were selected as study subjects.They were divided into IDA group(55 cases)and TM group(45 cases)according to type of anemia.Blood routine indicators[red blood cell count(RBC),white blood cell count(WBC),red blood cell distribution width(RDW),mean corpuscular volume(MCV),mean platelet volume(MPV),hemoglobin(Hb),mean corpuscular hemoglobin concentration(MCHC),mean corpuscular hemoglobin(MCH)and reticulocyte hemoglobin equivalent(RET-He)]were detected using fully automatic hematology analyzer.Above indicators were compared between the two groups.Multivariate Logistic regression analysis was conducted to identify the independent influencing factors for differential diagnosis of IDA and TM.Receiver operator characteristic curve(ROC curve)was plotted,and the area under ROC curve(AUC)was calculated to evaluate the clinical diagnostic efficacy of blood routine indicators for type of anemia.Results The levels of RBC,RDW,MCV and RET-He in IDA group were higher than those in TM group,and the levels of Hb and MCHC were lower than those in TM group[RBC(×1012/L):4.64±1.15 vs.2.79±0.81;RDW:(18.36±1.79)%vs.(12.23±1.48)%;MCV(fL):74.68±2.47 vs.66.89±2.42;RET-He(pg):20.43±5.62 vs.15.93±5.38;Hb(g/L):81.41±12.07 vs.107.65±12.41;MCHC(g/L):310.56±13.41 vs.321.45±14.68;all P<0.05].Logistic regression analysis results showed that RDW,MCV and Hb were independent influencing factors for differential diagnosis of IDA and TM,and the AUC values were 0.840,0.836 and 0.855.The AUC of combination of three indicators was 0.915,and the sensitivity and specificity were 88.58%and 90.02%.Conclusion Blood routine test is beneficial to distinguish IDA and TM,and the diagnostic accuracy is high.
