耳后裂纹性棘皮瘤1例
A case of retroauricular acanthoma fissuratum

摘要
29岁女性患者,右耳后斑块9年余.患者右侧耳后可见一个3 cm×0.8 cm斑块,质中,斑块中央表面粗糙,可见一线状凹痕,凹痕与眼镜架位置相对应.皮损组织病理:表皮角化过度伴角化不全,局部角质层呈裂隙状改变,棘层轻度水肿伴增生肥厚,真皮浅中层可见淋巴细胞及组织细胞浸润.诊断:裂纹性棘皮瘤.治疗:将眼镜架包裹,皮损处予尿素软膏、糠酸莫米松乳膏外用,8周后皮损完全消退.随访5个月,皮损无复发.
Abstract
A 29-year-old female patient complained of a right retroauricular plaque for more than 9 years.A plaque of 3 cm×0.8 cm in size could be seen behind the patient's right ear,displaying a medium texture and a rough central surface.A linear indentation also visible,corresponding to the position of the spectacle frame.Histologic findings showed hyperkeratosis,parakeratosis,localized fissure-like changes in the stratum corneum and acanthosis with mild edema.Additionally,there were lymphocytic and histiocytic infiltrations in the superficial middle layer of the dermis.The patient was instructed to wrap the spectacle frame,and apply topical glucocorticoids and urea ointment.After 8 weeks,the skin lesions completely subsided.No recurrence was observed during the 5 months follow-up.
