A case of primary cutaneous nocardiosis caused by Nocardia brasiliensis in an immunocompetent patient was reported. A 68-year-old male patient presented with redness,swelling,ulcers and pain on the left lower limb for 7 days. Dermatological examination showed that redness and swelling were observed in the left lower limb,with tenderness and increased skin temperature. Two areas of skin ulceration with purulent and serosanguinous exudate were noted over the left lateral malleolus. Scattered dark red patches were present on the anterior shin,with the skin feeling indurated. The results of routine bacterial culture,identification,and mass spectrometry analysis ultimately revealed infection with Nocardia brasiliensis. The diagnosis of primary cutaneous nocardiosis caused by Nocardia brasiliensis was made.The wound improved following abscess incision and drainage,along with anti-infection treatment with sulfamethoxazole and trimethoprim.The patient claimed that the wound recovered well and there was no recurrence after 6 months of follow-up.
