摘要

目的 观察高强度聚焦超声(HIFU)联合三维适形放疗(3DCRT)治疗晚期原发性肝癌近期疗效.方法 对41例晚期肝癌给予3 DC RT治疗,单次剂量3 Gy/次,3~4次/周,总剂量39 ~60 Gy,中位总剂量达52 Gy,4~5周完成.同步给予HIFU,平均接受治疗6次(4~10次).结果 41例患者均可评价疗效,肿瘤体积变化CR 2例、PR 27例、NC 9例、PD 3例.总有效率MR 70.73%( CR+ PR).1年、2年生存率分别为75.60% (31/41)、53.66%( 22/41),中位TTP14.6个月.临床受益指数92.68%( 38/41).结论 HIFU+ 3DCRT方案同步治疗晚期原发性肝癌可适当延长生存期,提高生活质量,无明显的不良反应.

Abstract

Objective To observe the short term efficacy of high intensity focused ultrasound ( HIFU) combined with three dimensional conformal radiotherapy (3 D-CRT) on primary hepatocellular carcinoma ( PLC). Methods HIFU and 3D-CRT were performed in 41 patients with PHC. In 3D-CRT treatment,the patients were irradiated with a fraction of 3 Gy,and 3 to 4 fractions (dose of 39 -60 Gy) a week,and total to 4 to 5 weeks. While the HIFU treatment was 4 to 10( mean 6) fractions. Results Forty-one patients were evaluable for response,in which 2 case complete response(CR) in the volume of tumor,27 case partial re-sponse( PR) ,9 case no change(NC) and 3 case progressive disease( PD),namely the MR was 70.73%. The 1-and 2-year survival were 75. 60% ,53.66% respectively and foe all patients the mediam survival period was 14.6 months. The clinical beneficial indice was 92.68%. Conclusion HIFU in combinatiom with 3D-CRT is a safe,minor-invasive treatment. It was proved to be effective in alleviating the sufferings, improving clinical beneficial indices and extending life circle in PLC.
