替吉奥胶囊联合三维适形放疗在胃癌进展期的应用效果

摘要
目的 观察替吉奥胶囊联合三维适行放疗技术在胃癌进展期患者的临床效果及毒副作用.方法 选择东阳市人民医院2010年4月-2013年4月收治的142例确诊为胃癌进展期患者,按照所在病区分为单纯放疗组69例与放化疗结合组73例,单纯放疗组患者单纯采用三维适行放疗法进行放射治疗,180 cGy/d,5次/周,照射4～8周共18～30次,总剂量4500～5500 cGy.放化疗结合组患者在此基础上给予抗肿瘤药物替吉奥胶囊口服治疗,40 mg/m2,每天早晚饭后各1次,连服5d后休2d,7d为1个周期,共进行4个周期.比较2组患者治疗后的临床疗效及毒副作用.结果 治疗后放化疗结合组患者CR 26例,PR 20例,临床总有效为63.01％,单纯放疗组患者CR 11例,PR 22例,临床总有效率47.83％,2组比较差异有统计学意义(P＜0.05);不良反应方面,虽放化疗组患者Ⅱ～Ⅳ期血液学毒性方面毒副作用较多,但均在患者耐受内,且经过对症治疗及相应支持后症状迅速缓解,未出现严重后果.结论 替吉奥胶囊联合三维适形放疗可以有效改善患者临床疗效,虽有一定毒副作用,但是均在患者临床耐受内,两者联合值得在临床广泛推广应用.
Abstract
Objective To observe the clinical effects and side effects of Tegafur capsules combined with three-dimensional conformal radiotherapy in patients with advanced gastric cancer.Methods Total 142 cases of patients with advanced gastric cancer in our hospital from April 2010 to April 2013 were divided into radiotherapy group (69 cases) and chemotherapy group(73 cases) according to different wards.The radiotherapy group were given three-dimensional conformal radiotherapy,180 cGy/d,5 times/w,totally 4-8 weeks and 18-30 times,totally dose were 4500-5500 cGy;on the basis of it,the chemotherapy group added Tegafur capsules,40 mg/m2,1 time after breakfast and dinner per day,continuous 5 days and rested 2 days,7 days was a circle,totally 4 circles.The clinical efficacy and side effects between the two groups were compared.Results After the treatment,totally clinical effective rate in the chemotherapy group was 63.01％ (CR 26cases,PR 20 cases),which was higher than that in the radiotherapy group(47.83％,CR 11 cases,PR 22 cases),the difference was statistically significant(P ＜ 0.05).Although the side effects of Ⅱ-Ⅳ hematologic toxicity chemotherapy in the chemotherapy group were more,but all in patients tolerated;and after symptomatic treatment and impact mitigation support,there were not appear serious consequences.Conclusion Tegafur capsules combined with three-dimensional conformal radiotherapy can effectively improve the clinical symptoms,and the effects are all in patients tolerated,which is widely used in clinical application.
