摘要

目的 基于营养风险筛查2002(NRS2002)系统分析2型糖尿病患者的营养状况,研究患者营养风险的主要影响因素.方法 选取2022年10月10 日-2023年5月9日亳州市人民医院收治的糖尿病住院患者196例,根据有无营养风险分为有营养风险组(35例)和无营养风险组(161例).分析患者的性别和相关生化指标(血红蛋白、糖化血红蛋白、白蛋白、总胆固醇、甘油三酯、尿素氮和肌酐)在有、无营养风险患者中的差异,采用多元lo-gistic 回归分析研究营养风险的独立影响因素.结果 不同性别患者营养风险的发生差异无统计学意义(P>0.05).相关临床生化检测结果显示,有营养风险的T2DM患者血清血红蛋白、白蛋白、尿素氮、肌酐分别为(122.29±10.07)g/L、(40.98±3.61)g/L、(6.97±0.16)mmol/L、(86.91±5.96)mmol/L,与无营养风险患者比较差异均有统计学意义[(128.15±9.96)g/L、(43.38±4.34)g/L、(5.81±0.92)mmol/L、(79.75±8.56)mmol/L,P<0.05];有、无营养风险患者糖化血红蛋白、总胆固醇及血清甘油三酯比较差异均无统计学意义(P>0.05).多元logistic回归分析显示,血红蛋白、白蛋白和肌酐是T2DM患者发生营养风险的影响因素.结论 应用NRS2002评估T2DM患者营养状况,血红蛋白、白蛋白和肌酐水平是T2DM患者发生营养风险的影响因素.

Abstract

Objective To systematically analyze the nutritional status of type 2 diabetes(T2DM)patients based on nu-trition risk screening(NRS2002),and screen the main impact indicators of nutritional risk.Methods A total of 196 T2DM patients were obtained from the People's Hospital of Bozhou between October 10,2022 and May 9,2023.The pa-tients were divided into nutritional risk group(35 cases)and non-nutritional risk group(161 cases)based on the pres-ence or absence of nutritional risk.Statistical analysis was used to investigate the differences in gender and related bio-chemical indicators(hemoglobin,glycated hemoglobin,albumin,total cholesterol,triglycerides,urea nitrogen,creati-nine)between patients with and without nutritional risk.The independent influencing factors of nutritional risk were ana-lyzed using multiple logistic regression analysis.Results There was no significant difference in the occurrence of nutri-tional risks between different genders(P>0.05).The relevant clinical biochemical test results showed that T2DM pa-tients with nutritional risk had significant differences in serum hemoglobin[(122.29±10.07)g/L],albumin[(40.98± 3.61)g/L],urea nitrogen[(6.97±0.16)mmol/L],and creatinine[(86.91±5.96)mmol/L]levels compared to non-nutritional risk patients[(128.15±9.96)g/L,(43.38±4.34)g/L,(5.81±0.92)mmol/L,(79.75±8.56)mmol/L,respectively,P<0.05].There was no significant difference in the statistics of glycated hemoglobin,total cho-lesterol,and serum triglycerides between patients with and without nutritional risk(P>0.05).Multiple logistic regres-sion analysis was used to analyze hemoglobin,albumin,urea nitrogen,and creatinine.The results showed that hemoglo-bin,albumin,and creatinine were related risk factors for nutritional risk in T2DM patients.Conclusion NRS2002 is applied to assess the nutritional status of patients with T2DM,and haemoglobin,albumin and creatinine levels are influ-ential factors in die occurrence of nutritional risk in patients with T2DM.
