摘要
目的 分析负压封闭引流技术(Vacuum sealing drainage,VSD)联合中厚皮片移植治疗糖尿病足慢性溃疡创面的临床效果.方法 选取山东省阳谷县人民医院2022年4月-2023年1月收治的80例糖尿病足慢性溃疡患者作为研究对象,采取随机数字表法将患者分为对照组(n=40)和观察组(n=40),对照组实施常规中厚皮片移植治疗,观察组在对照组的基础上联合应用VSD治疗.比较两组患者临床指标、溃疡面积缩小率、肉芽组织生长情况、疼痛程度及瘢痕情况.结果 观察组术后换药次数少于对照组,住院时间、创面愈合时间短于对照组,皮片成活率高于对照组(P<0.05).观察组治疗2周和4周后溃疡面积缩小率及肉芽组织覆盖率均高于对照组(P<0.05).观察组治疗3个月、6个月后VAS与VSS评分均低于对照组(P<0.05).结论 在糖尿病足慢性溃疡患者中应用VSD联合中厚皮片移植治疗效果显著,能够有效减少换药次数,促进创面愈合,提高皮片移植成活率,缩短住院时间、创面愈合时间,并降低患者疼痛及瘢痕情况.
Abstract
Objective To analyze the clinical effect of vacuum sealing drainage(VSD)combined with medium thickness skin graft transplantation on chronic ulcer wounds of Diabetic foot.Methods The 80 patients with chronic ulcer of Diabetic foot who were admitted to Yanggu County People's Hospital of Shandong Province from April 2022 to Jan 2023 were selected as research objects.The patients were randomly divided into control group(n=40)and observation group(n=40).Compared the clinical indicators,ulcer area reduction rate,granulation tissue growth,pain level,and scar formation between two groups of patients.Results The observation group had fewer postoperative dressing changes than the control group,shorter hospital stay and wound healing time,and higher skin graft survival rate than the control group(P<0.05).After 2 and 4 weeks of treatment,the ulcer area reduction rate and granulation tissue growth in the observation group were higher than those in the control group(P<0.05).After 3 and 6 months of treatment,the VAS and VSS scores in the observation group were lower than those in the control group(P<0.05).Conclusion The application of VSD combined with medium thickness skin graft in the treatment of chronic ulcer in Diabetic foot patients has a significant effect,which can effectively reduce the number of dressing changes,promote wound healing,improve the survival rate of skin graft transplantation,shorten the hospital stay,wound healing time,and reduce pain and scar in patients.
