摘要
目的 探讨上抬甲板结合纱条填塞引流法治疗嵌甲型甲沟炎保守治疗的临床效果.方法 2019年1月-2022年12月在佛山市第五人民医院门诊接诊的120例142指(趾)嵌甲型甲沟炎患者采用上抬甲板结合纱条填塞引流法保守治疗.术后随访3～24个月,观察其临床治疗效果并进行功能评价.结果 120例142指(趾)均采用上抬甲板治疗,除2例伤口感染加重,其余甲沟炎伤口均甲级愈合.对所有患者进行随访,失访6例,114例获随访,随访时间3个月～2年,平均10.2个月,其中60例随访9个月以上,3例随访时间达2年.术后甲沟完整,外观正常,指(趾)甲正常生长,无嵌甲发生,甲面平整光滑.患指(趾)感觉及运动功能均正常,未出现并发症及后遗症等,无运动疼痛及不适感,达到痊愈标准.结论 采用上抬甲板结合纱条填塞引流法治疗嵌甲型甲沟炎方式简单,疗效显著,术后复发率低,值得临床推广应用.
Abstract
Objective To explore the clinical efficacy of the conservative treatment of ingrown nail-type paronychia using the method of lifting the nail plate and filling it with gauze for drainage.Methods The clinical data of 120 patients with ingrown nail-type paronychia admitted to the outpatient department of the Fifth People's Hospital of Foshan City from January 2019 to December 2022 were collected.All patients were treated with the method of lifting the nail plate and filling it with gauze for drainage.The postoperative observations included nail healing,appearance,and pain.Results A total of 142 fingers/toes in 120 cases were treated by lifting the nail plate,except for 2 cases where the wound became infected.The remaining paronychia wounds all healed to grade I.Follow-up was conducted for all patients,with 6 cases lost to follow-up.A total of 114 cases were followed up for a period of 3 months to 2 years,with an average of 10.2 months.Among them,60 cases were followed up for more than 9 months,and 3 cases were followed up for 2 years.After the operation,the paronychia was completely resolved,the appearance was normal,the nails grew normally without ingrowth,and the nail surface was smooth.The sensation and motor function of the affected fingers/toes were normal,and no complications or sequelae were observed.There was no pain or discomfort during movement,and the recovery criteria were met.Conclusion The method of lifting the nail plate and filling it with gauze for drainage is a simple and effective treatment for ingrown nail-type paronychia.It has a low postoperative recurrence rate and is worthy of clinical promotion and application.
