摘要
目的 探讨腓肠肌腱膜瓣翻转治疗陈旧性跟腱断裂的临床疗效.方法 对2019年2月-2023年3月收治的6例陈旧性跟腱断裂患者应用腓肠肌腱膜瓣翻转治疗.术中于小腿中部腓肠肌腱膜与跟腱移行处切取一条长8.0～12.0 cm、宽3.0 cm带蒂腓肠肌腱膜瓣,将腱膜瓣与跟腱远端用改良Kessler法缝合.结果 6例患者手术切口均一期愈合,并全部完成随访,随访时间6～15个月,平均(9.9±3.7)个月.按Arner-Lindholm标准评定:优5例,良1例,临床疗效良好.结论 腓肠肌腱膜瓣翻转治疗陈旧性跟腱断裂疗效优良.
Abstract
Objective To explore the clinical efficacy of gastrocnemius tendon flap reversal in the treatment of chronic Achilles tendon rupture.Methods A retrospective analysis was conducted on 6 patients with chronic Achilles tendon rupture treated with gastrocnemius tendon flap reversal from February 2019 to March 2023.During the surgery,a pedicled gastrocnemius tendon flap approximately 8.0-12.0 cm long and 3.0 cm wide was harvested from the junction of the gastrocnemius tendon and achilles tendon in the middle of the lower leg.The Achilles tendon tension was adjusted,and the flap was sutured to the distal Achilles tendon using a modified Kessler technique.Results All 6 patients had primary wound healing.All patients completed follow-up,with an average follow-up period of(9.9±3.7)months(range 6-15 months).According to the Amer-Lindholm evaluation criteria,5 cases were rated as excellent,and 1 case as good,indicating favorable clinical outcomes.Conclusion The gastrocnemius tendon flap reversal method demonstrates excellent efficacy in treating chronic Achilles tendon rupture.
