骨外露创面应用Masquelet技术自愈的临床观察

摘要
目的 探讨骨外露创面应用Masquelet骨水泥充填后自愈的临床表现及疗效观察.方法 2019年1月-2022年6月收治8例软组织缺损骨外露病例,缺损面积:2.0 cm×2.0 cm～10.0 cm×6.0 cm,创面达深筋膜下,部分骨质外露,一期行创面清创,带万古霉素的骨水泥充填创面,术后4～6周取出骨水泥,未行皮瓣或植皮治疗,仅行间断换药至创面愈合.结果 8例经过6～12个月随访,所有创面均自行愈合,未出现溃疡、窦道及创面渗液等现象.愈合时间最短6周,最长20周.结论 部分骨外露创面经Masquelet技术骨水泥充填后,未行皮瓣或植皮治疗,可自行愈合,为临床治疗复合组织缺损创面提供了新的思路.
Abstract
Objective To investigate the clinical effect of self-healing cases after bone cement filling of wounds with exposed bone.Methods From January 2019 to June 2022,8 patients of exposed bone with soft tissue defects were treated,the defect area ranged from 2.0 cm×2.0 cm to 10.0 cm×6.0 cm,the wounds were deep into the subfascia,and part of the bone was exposed.Debridement was carried at the first stage,and bone cement with vancomycin was used to fill the wound,and the bone cement were removed at 4-6 weeks after surgery.No flap or dermatoplasty was performed,only intermittent dressing changes were performed until the wounds healed.Results After 6～12 months of follow-up,all the wounds healed by themselves.There were no symptoms such as ulcer,sinus and exudate.The shortest healing time was 6 weeks and the longest time was 20 weeks.Conclusion Some wounds with exposed bone can heal by themselves without flap or dermatoplasty after filling with bone cement.
