Objective To explore the effect of repairing skin and soft tissue defect of hand and wrist with retrograde interosseous dorsal artery island flap and free anterolateral thigh flap. Methods Forty-one cases of skin and soft tissue defect of wrist and hand were randomly divided into observation group(19 cases were treated with retrograde interosseous dorsal artery island flap)and control group(22 cases were treated with free anterolateral thigh flap). The survival rate, complication rate, excellent and good rate of flap appearance and donor site injury were compared between two groups. Results All the patients were followed up for 6 months. All the flaps survived. In the observation group, one case of skin graft in donor site had blisters on the skin margin, and the wound healed after dressing change, and the donor area of the remaining patients healed well.In the observation group, the hair of the donor site was exuberant, and the flap hair was treated with laser hair removal. The appearance and texture of the other flaps were satisfactory, significantly better than the control group. Conclusion The excellent and good rate of the retrograde interosseous dorsal artery island flap is better than that of the control group(P<0.05),and there is no significant difference in the survival rate and wound healing between two groups(P>0.05).Retrograde interosseous dorsal artery island flap and free anterolateral thigh flap have high survival rate, can effectively repair the soft tissue defect of wrist and hand.
