Objective To explore curative effects of repairing the skin defects of the calf,ankle and foot by free anterolateral thigh flap.Methods From June 2019 to June 2023,7 cases of the skin defects of the calf,ankle and foot were treated by free anterolateral thigh flap.Among them,there were 5 cases of foot wounds,1 case of ankle wounds,1 case of calf wounds.The size of defects ranged from 7 cm×8 cm to 10 cm ×21 cm.Observed the postoperative skin flap survival,followed up the recovery of skin flap appearance,motor sention,and donor site injuries.Results All the free anterolateral thigh flaps survived after surgery.The cutting area of two flaps was too large,resulting in the inability of the donor site incision to suture directly.Took a free graft from the opposite thigh for the donor site.All the other flaps donor areas can be directly sutured.All the free anterolateral thigh flaps were followed up after surgery.The period was 10 to 17 months(average13 months).At the last follow-up,the flaps had good circulation,soft texture,and good elasticity.All the medical records showed no occurrence of infection,hematoma,or related complications of donor site.According to the Foot and Ankle score standard,4 cases were excellent,2 cases were good,and 1 case was fair.According to the sensation score standard,4 cases were S2,3 cases were S3.Conclusion The anatomy of the perforating vessels in the anteriorlateral thigh flap remains relatively constant.The flap has a large cutting area and a fine quality.It is a better donor site for repairing skin defects in the calf、ankle and foot areas.It is worthy of further promotion in clinical practice.
