The effect of single skin pedicle V-Y advancement flap combined with pedicle skin grafting on hand function and joint range of motion in patients with fingertip defect
Objective To analyze the repair effect of single skin pedicle V-Y advancement flap combined with pedicle skin grafting on patients with finger tip defects.Methods A retrospective study was conducted on the data of 53 patients(53 fingers)with fingertip defects admitted to the hospital from January 2021 to March 2023.They were divided into two groups according to different treatment methods.Group A consisted of 22 patients(22 fingers)who were treated with a single subcutaneous pedicle V-Y advancement flap combined with pedicle skin grafting;Group B:31 patients(31 fingers)underwent reverse digital palmar proper artery island flap repair;Compared two groups of hand function,joint function activity,and peripheral blood circulation.Results Two groups of patients were followed up for 6-12 months after surgery,and both flaps and skin grafts survived without pain or infection in the incision.The flap sensation recovered well,without hook nail deformity,venous reflux obstruction,finger flexion contracture,deformity,etc.There was no difference in the postoperative recovery rate between two groups(P>0.05).At the 6th month after surgery,the overall hand function,daily living ability,work performance,appearance,and functional satisfaction of Group A were higher than those of Group B,with lower hand pain scores and higher degree of autonomous motion of finger joints compared to Group B.The local tissue oxygen partial pressure(TcPO2)and blood perfusion volume of the affected fingers were higher than those of Group B(P<0.05).Conclusion Single skin pedicle V-Y advancement flap combined with pedicle skin grafting can promote hand function recovery,increase joint mobility,and has no obvious complications in patients with fingertip defects,making it worth applying.
