摘要
目的 评估人乳头瘤病毒基因(HPV-DNA)分型检测对宫颈癌前病变的筛查临床价值.方法 回顾性分析2021年2月至2023年2月洛阳市东方人民医院收治的宫颈疾病患者的临床资料,依据不同的疾病类型将其分为试验组1(宫颈炎患者100例)、试验组2[宫颈上皮内瘤变(CIN)Ⅰ级60例]、试验组3(CIN Ⅱ级患者60例)、试验组4(CIN Ⅲ级患者60例)、试验组5(宫颈癌患者80例).比较5组受试者各高危亚型HPV DNA表达水平和阳性率,比较5组患者HPV DNA分型检测阳性率,Logistic回归分析HPV DNA分型与宫颈癌以及癌前病变的相关性.结果 5组患者高危亚型HPV DNA水平均差异有统计学意义(P<0.05).5组患者高危亚型HPV DNA阳性率比较,组间差异具有统计学意义(P<0.05).5组患者HPV DNA分型检测总阳性率比较,组间差异具有统计学意义(P<0.05).Logistic回归分析显示HPV DNA分型检测16、18、31、33、35、45、51、52、56、58型阳性是患者宫颈癌及癌前病变发生的独立危险因素[OR值(95%CI)=11.024(4.987,23.056)、8.759(2.896,15.427)、5.461(2.147,11.322)、5.132(2.642,11.124)、1.321(1.167,5.139)、1.124(1.072,3.251)、5.513(5.234,9.635)、5.574(2.127,11.349)、1.796(1.618,3.156)、6.673(3.643,8.869),P<0.05].结论 HPV-DNA 分型检测有利于宫颈癌及宫颈癌癌前病变的病情判断.
Abstract
Objective To evaluate the clinical value of human papilloma virus DNA typing detection in screening cervical precancerous lesions.Methods Clinical data of cervical disease patients admitted to Dongfang People's Hospital in Luoyang from February 2021 to February 2023 were retrospectively analyzed.They were divided into experimental group 1(100 patients with cervicitis),experimental group 2(60 patients with cervical intraepithelial neoplasia(CIN)grade Ⅰ),experimental group 3(60 patients with CIN grade Ⅱ),experimental group 4(60 patients with CIN grade Ⅲ),and experimental group 5(80 patients with cervical cancer)based on different disease types.We compare the expression levels and positive rates of HPV DNA in each high-risk subtype of 5 groups of subjects,the positive rates of HPV DNA typing detection in 5 groups of patients,and analyze the correlation between HPV DNA typing and cervical cancer and precancerous lesions through logistic regression analysis.Results There was a significant difference in the DNA levels of high-risk subtypes of HPV among the five groups of patients(P<0.05).There was a statistically significant difference in the positive rates of high-risk subtypes of HPV DNA among 5 groups of patients(P<0.05).There was a statistically significant difference in the total positive rate of HPV DNA typing among the 5 groups of patients(P<0.05).Logistic regression analysis showed that positive HPV DNA typing tests for types 16,18,31,33,35.45.51,52,56,and 58 were independent risk factors for cervical cancer and precancerous lesions in patient.s[OR(95% CI)=1 1.024(4.987,23.056)、8.759(2.896,15.427)、5.461(2.147,11.322)、5.132(2.642,11.124)、1.321(1.167,5.139)、1.124(1.072,3.251)、5.513(5.234,9.635)、5.574(2.127,11.349)、1.796(1.618,3.156)、6.673(3.643,8.869),P<0.05].Conclusion HPV-DNA typing detection is conducive to the diagnosis of cervical cancer and precancerous lesions of cervical cancer.
