The predictive efficacy of preoperative C-reactive protein/albumin expression on the recurrence of triple negative breast cancer after breast conserving surgery
Objective To analyze the predictive efficacy of preoperative C-reactive protein/albumin(CAR)on the recurrence of triple negative breast cancer(TNBC)after breast conserving surgery.Methods The clinical data of 109 TNBC patients who received breast conserving surgery in our hospital from January 2021 to May 2022 were retrospectively analyzed.The serum tumor marker levels and CAR changes of the enrolled patients before and after surgery were compared.The correlation between CAR and tumor marker levels of TNBC patients was tested by Spearman correlation coefficient.According to whether the postoperative condition relapses,the enrolled patients are divided into a recurrent group and a non-recurrent group.The general and clinical data of the two groups are compared,and the risk factors that may lead to the recurrence of TNBC after surgery are summarized through logistic multiple factor regression analysis.The predictive efficacy of each risk factor on the recurrence of TNBC after breast conserving surgery is verified by drawing the ROC and observing the AUC.Results The quartiles of postoperative CA153,CA199,CA125,CEA,and CAR in 109 patients were(27±5)U/ml,(40±5)U/ml,(41±5)U/ml,(5.1±1.2)ng/ml and(0.92±0.24),respectively,lower than those before surgery[(31±5)U/ml,(43±5)U/ml,(44±5)U/ml,(7.6±2.0)ng/ml and(1.71±0.35)](P<0.05).Spearman correlation coefficient test showed a positive correlation between CAR quartiles and serum tumor marker levels in TNBC patients.Age,the proportion of tumor TNM stage Ⅱ,and preoperative CAR quartile of the recurrent group were(52±10)years old,86.2%(25/29)and(1.83±0.41),respectively,which were higher than those of the non-recurrent group[(47±10)years old,55%(44/80)and(1.33±0.26)](P<0.05).Logistic multivariate regression analysis showed that age,TNM staging and CAR were risk factors for recurrence of TNBC after breast conserving surgery(P<0.05).The ROC curve shows that CAR has a high predictive efficacy on the recurrence of TNBC after breast conserving surgery(AUC>0.85).Conclusion There is a significant positive correlation between preoperative CAR and serum tumor marker levels in TNBC patients,and preoperative monitoring of CAR can to some extent achieve early prediction of recurrence after breast conserving surgery.
