High-frequency color Doppler ultrasound in preoperative target vessel selection testing for autologous arteriovenous endovascular fistulae in maintenance hemodialysis patients with end-stage renal disease
Objective To explore the clinical value of using high-frequency color Doppler ultrasound in preoperative target vessel selection testing of autologous arteriovenous endovascular fistulas in maintenance hemodialysis patients with end-stage renal disease. Methods The clinical data of 103 patients with end-stage renal disease who received maintenance hemodialysis treatment with autologous arteriovenous fistula from August 2021 to June 2023 were retrospectively analyzed, and they were divided into the control group(no,n=51)and the observation group(yes,n=52)according to whether or not high-frequency color Doppler ultrasonography was used to detect the selection of target blood vessels before the operation, and statistics were collected on the number and time of successful operation and repair of the fistula. The two groups were divided into the control group and the observation group according to whether the target vessel selection was detected by high-frequency color Doppler ultrasound before surgery. Results The success rate of initial operation in the observation group was higher than that in the control group(P<0.05).The operation time of the observation group was shorter than that of the control group(P<0.05).The blood flow of internal fistula, lumen diameter and length of vessels available for puncture were greater in the observation group than in the control group(P<0.05).The subcutaneous depth of vessels in the observation group was smaller than that in the control group(P<0.05).The postoperative autologous arteriovenous endovascular fistula maturation attainment rate of the observation group was higher than that of the control group(P<0.05).The internal diameter and blood flow velocity of radial artery and cephalic vein in the observation group after surgery were greater than those before surgery(P<0.05).Conclusion High-frequency color Doppler ultrasound can accurately define the vessel alignment, diameter, and blood flow information in the preoperative period of autologous arteriovenous endovascular fistulae in patients with end-stage renal disease on maintenance hemodialysis, which can provide an effective reference for selecting the appropriate vessel location for the surgery and vessel anastomosis, improve the success rate of the one-time surgery, reduce the chances of fistula repair again, and be more conducive to the maturation of the autologous arteriovenous fistulae in the postoperative period.
