Objective To explore the risk factors of intervertebral disc injury in patients with traumatic thoracolumbar fractures based on CT imaging parameters.Methods The clinical data of 60 patients with traumatic thoracolumbar fractures were retrospectively analyzed in our hospital from January 2021 to December 2023.According to the intervertebral disc injury,the patients were divided into study group(patients with intervertebral disc injury,n=30)and control group(patients without intervertebral disc injury,n=30).The general clinical data and CT imaging parameters(intervertebral space height,spinal canal soft tissue invasion rate,spinal nerve root canal soft tissue invasion rate,kyphosis Cobb angle,posterior ligament complex injury,affected vertebral endplate angle,vertebral translation distance,interspinous process distance,interpediculate distance)were compared between groups.The risk factors of intervertebral disc injury in patients with traumatic thoracolumbar fractures were analyzed by unconditional logistic multivariate stepwise regression analysis.Results There were no statistical differences in the soft tissue invasion rate of spinal nerve root canal and injury rate of posterior ligament complex between the two groups(P>0.05).The height of intervertebral space and soft tissue invasion rate of spinal canal in study group were lower than those in control group(P<0.05),while the kyphosis Cobb angle,posterior ligament complex injury,affected vertebral endplate angle,vertebral translation distance,interspinous process distance and interpediculate distance were higher than those in control group(P<0.05).Unconditional logistic multivariate stepwise regression analysis showed that affected vertebral endplate angle and vertebral translation distance were not independent risk factors for intervertebral disc injury in patients with traumatic thoracolumbar fractures(P>0.05).Intervertebral space height,spinal soft tissue invasion rate,kyphosis Cobb angle,interspinous process distance and interpediculate distance were independent risk factors for intervertebral disc injury in patients with traumatic thoracolumbar fractures(P<0.05).Conclusion Intervertebral space height loss,spinal canal soft tissue invasion,kyphosis,interspinous process distance increase and interpediculate distance increase are all risk factors for intervertebral disc injury in patients with traumatic thoracolumbar fractures.The clinical application of CT imaging parameters can provide a favorable reference for the diagnosis and treatment of patients with thoracolumbar fractures.
