Correlation analysis of ultrasound parameters with myocardial injury markers in elderly patients with pulmonary heart disease complicated with coronary heart disease
Objective To analyze the correlation between ultrasonic parameters and myocardial injury markers in elderly patients with pulmonary heart disease(CPHD)complicated with coronary heart disease(CHD).Methods A total of 200 elderly CPHD patients from the Department of Geriatric Medicine,Jinshui District General Hospital of Zhengzhou City from November 2022 to November 2023 were selected and divided into two groups according to whether they had CHD or not.Eighty patients with CHD were set as observation group,and 120 patients without CHD were set as comparison group.All patients were given cardiac color doppler ultrasonography,and fasting venous blood was collected to detect myocardial injury markers.Cardiac color Doppler ultrasonography parameters(left ventricular end-systolic diameter(LVESD),end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)and myocardial injury markers[troponin Ⅰ(cTnⅠ)and creatine kinase isoenzyme(CK-MB)]of the two groups were compared.Cardiac color doppler ultrasound parameters and myocardial injury markers in different New York Heart Association(NYHA)classification groups in elderly CPHD patients with CHD were compared,and the correlation between cardiac color Doppler ultrasound parameters and myocardial injury markers was analyzed by Pearson correlation aralysis method.Results LVESD and LVEDD in observation group were higher than those in comparison group(P<0.05),and LVEF in observation group was lower than that in comparison group(P<0.001).The levels of serum cTnⅠ and CK-MB in observation group were higher than those in control group(P<0.001).LVESD and LVEDD in grade Ⅲ~Ⅳ group were higher than those in gradeⅠ~Ⅱ group(P<0.001),and LVEF in grade Ⅲ~Ⅳ group was lower than that in grade Ⅰ~Ⅱ group(P<0.001).The levels of serum cTnⅠ and CK-MB in grade Ⅲ~Ⅳ group were higher than those in grade Ⅰ~Ⅱ group(P<0.001).LVESD and LVEDD were positively correlated with cTnⅠ and CK-MB(r=0.382,0.391,0.376,0.390,all P<0.001).And LVEF was negatively correlated with cTnⅠ and CK-MB(r=-0.371,-0.383,P<0.001).Conclusion The levels of LVESD,LVEDD and serum cTnⅠ and CK-MB in elderly CPHD patients with CHD are higher,while the levels of LVEF are lower.LVESD,LVEDD and LVEF are correlated with cTnⅠ and CK-MB to a certain extent,which can assist clinical evaluation of patients'disease.
