可视双腔管导管在肺隔离手术中的应用

摘要
探讨可视双腔管导管(DLT)在肺隔离手术中的应用效果.方法 按随机数字表法将2019年3月至2022年3月于我院进行肺隔离手术的100例患者分为2组,每组各50例.观察组采用可视DLT进行肺隔离手术,对照组采用普通DLT进行肺隔离手术.比较2组DLT插管时间、导管定位时间、血流动力学及并发症.结果 观察组DLT插管时间为(41±11)s、导管定位时间为(81±30)s,均短于对照组的(51±13)s、(149±61)s,差异有统计学意义(P＜0.05);对照组气管插管后5 min(T1)、手术结束(T2)时心率(HR)均高于气管插管前(T0),观察组T1时HR高于T0,2组T1、T2时平均动脉压(MAP)均高于T0,观察组T1、T2时HR、MAP均低于对照组;观察组并发症总发生率(4％)低于对照组(24％),差异有统计学意义(x2=8.306,P＜0.05).结论 可视DLT用于肺隔离手术中是安全可行的,插管时间及导管定位时间短,有利于稳定患者的血流动力学,减少并发症发生.
Abstract
Objective To explore the application effect of visual double lumen catheter(DLT)in lung isolation surgery.Methods Using the random number table method,100 patients who underwent lung isolation surgery in our hospital from March 2019 to March 2022 were divided into two groups,50 cases in each group.The observation group was treated with visual DLT for lung isolation surgery,and the control group was treated with ordinary DLT for lung isolation surgery.DLT intubation time,catheter positioning time,hemodynamics and complications were compared between the two groups.Results The DLT intubation time and catheter positioning time of the observation group were(41±11)s and(81±30)s,which were shorter than those of the control group(51±13)s and(149±61)s,respectively,and the difference was statistically significant(P＜0.05).The heart rate(HR)of the control group at T1 and T2 was higher than that of T0,the HR of the observation group at T1 was higher than T0,the mean arterial pressure(MAP)of the two groups at T1 and T2 was higher than that at T0,the HR and MAP of the observation group at T1 and T2 were lower than those of the control group,the total incidence of complications of the observation group(4％)was lower than that of the control group(24％),the difference was statistically significant(x2=8.306,P＜0.05).Conclusion Visual DLT is safe and feasible in lung isolation surgery,with short intubation time and catheter positioning time,which is conducive to stabilizing the hemodynamics of patients and reducing complications.
