摘要
目的 探讨应用智能盆底超声评价不同产次及分娩方式对产后盆底功能的影响.方法 选取2022年5月至2023年12月在我院进行产后盆底超声检查的产妇350例,其中,初次顺产150例,二次顺产100例,初产剖宫产100例.比较初次顺产组与二次顺产组,初次顺产组与剖宫产组的二维及三维盆底超声相关参数指标的差异.结果 初次顺产组与二次顺产组其静息膀胱后角,Valsalva状态下膀胱颈距离、膀胱颈移动度、收缩状态肛提肌裂孔前后径及周长的比较差异具有统计学意义(P<0.05);而初次顺产组与剖宫产组其静息状态下的肛提肌板角度、膀胱颈距离,Valsalva状态下的膀胱颈距离、膀胱颈移动度、尿道倾斜角、尿道旋转角、尿道旋转角、膀胱后角开放、膀胱后角度数、子宫颈距离、直肠壶腹部距离及所有盆底三维参数指标的比较差异均具有统计学意义(P<0.05).结论 智能盆底超声可评价产后早期盆底结构功能的变化,为后续的产后盆底康复治疗提供一定的影像学依据,且顺产者对其产后早期盆底结构功能影响较大.
Abstract
Objective To evaluate the effect of different delivery times and delivery modes on postpartum pelvic floor function by using smart pelvic floor ultrasound.Methods Three hundred and fifty cases of puerperal pelvic floor ultrasonography were selected in our hospital,including 150 cases of first vaginal delivery,100 cases of second vaginal delivery and 100 cases of cesarean section.The difference of two-and three-dimensional pelvic floor ultrasound parameters between the first vaginal birth group and the second vaginal birth group,the first vaginal birth group and the cesarean section group was compared.Results Significant differences were observed in posterior bladder angle at rest,bladder neck distance,and bladder neck mobility during Valsalva maneuver,as well as anteroposterior diameter and circumference of levator hiatus during contraction between the primary and secondary natural labor groups(P<0.05).However,significant differences were observed in levator plate angle and bladder neck distance at rest,bladder neck distance,bladder neck mobility,urethral inclination angle,urethral rotation angle,posterior bladder Angle opening,posterior bladder Angle number,cervical distance,rectal ampulla distance during Valsalva maneuver and all pelvic floor 3D parameter indicators between the primary natural labor group and the cesarean section group(P<0.05).Conclusion Smart pelvic floor ultrasound can evaluate the changes of pelvic floor structure and function in the early postpartum period,providing certain imaging basis for subsequent postpartum pelvic floor rehabilitation treatment,and vaginal delivery has a greater impact on its pelvic floor structure and function in the early postpartum period.
