摘要
目的 研究超声造影联合血清肿瘤标志物[糖类抗原125(CA125)、血管内皮生长因子(VEGF)、基质金属蛋白酶9(MMP-9)]检测在子宫内膜癌诊断中的应用价值.方法 选择2022年9月至2023年9月住院65例子宫内膜癌患者,同期65例子宫内膜良性病变患者,进行超声造影检查和血清CA125、VEGF、MMP-9检测,比较2组检测结果,分析超声造影联合血清CA125、VEGF、MMP-9检测在子宫内膜癌诊断中应用价值.结果 子宫内膜癌组峰值强度大于子宫内膜良性病变组,达峰时间、上升时间短于子宫内膜良性病变组,差异有统计学意义(P＜0.05);子宫内膜癌组血清CA125、VEGF、MMP-9高于子宫内膜良性病变组,差异有统计学意义(P＜0.05);经logistic回归分析显示,超声造影参数峰值强度、达峰时间、上升时间及血清CA125、VEGF、MMP-9水平与子宫内膜癌有关(P＜0.05).绘制受试者工作特征(ROC)曲线分析显示,超声造影参数峰值强度、达峰时间、上升时间及血清CA125、VEGF、MMP-9水平诊断子宫内膜癌的曲线下面积分别为:0.860、0.885、0.891、0.913、0.861、0.891;联合检测曲线下面积最高,为0.957.结论 超声造影联合血清CA125、VEGF、MMP9检测在子宫内膜癌诊断中应用价值较高.
Abstract
Objective To explore the application value of contrast-enhanced ultrasound combined with serum carbohydrate antigen 125(CA125),vascular endothelial growth factor(VEGF),and matrix metalloproteinase-9(MMP-9)detection in the diagnosis of endometrial cancer.Methods Sixty-five patients with endometrial cancer and 65 patients with benign endometrial lesions were selected from September 2022 to September 2023 for contrast-enhanced ultrasound examination and serum CA125,VEGF,and MMP-9 detection.The results of the two groups were compared,and the application value of contrast-enhanced ultrasound combined with serum CA125,VEGF,and MMP-9 detection in the diagnosis of endometrial cancer was analyzed.Results The peak intensity of endometrial cancer group was higher than that of benign endometrial disease group,and the peak and rise times were shorter than those of benign endometrial disease group,with statistical significance(P＜0.05).The serum levels of CA125,VEGF,and MMP-9 in the endometrial cancer group were higher than those in the benign endometrial lesions group,and the differences were statistically significant(P＜0.05).Through logistic regression analysis,the results showed that the peak intensity,peak time,rise time of contrast-enhanced ultrasound,as well as serum levels of CA125,VEGF,and MMP-9,were associated with endometrial cancer(P＜0.05).The ROC curve analysis showed that the area under the curve for diagnosing endometrial cancer using contrast-enhanced ultrasound parameters such as peak intensity,time to peak,rise time,and serum levels of CA125,VEGF,and MMP-9 were 0.860,0.885,0.891,0.913,0.861,and 0.891,respectively.The area under the curve for combined detection was the highest,which was 0.957.Conclusion The combination of contrast-enhanced ultrasound and serum levels of CA125,VEGF,and MMP-9 has high diagnostic value in endometrial cancer.
