摘要
分析颞叶癫痫患者发作间隙期的脑电生理信号特征,探究其与抑郁症状的相关性.方法 选取2022年1月至2023年6月在郑州市第二人民医院神经内科接受手术治疗的癫痫患者200例.根据汉密尔顿抑郁量表(HAMD-17)分为抑郁组和非抑郁组.通过立体定向脑电图和汉密尔顿抑郁量表,采用Pearson相关分析颞叶癫痫发作间歇期脑电生理信号特征及和抑郁症状间的相关性.结果HAMD-17量表总分与颞中回前部的波幅绝对平均值、波幅标准方差、全频段总功率、θ功率及δ功率相关系数显著相关(r=0.503,P=0.013;r=0.492.P=0.014;r=0.423,P=0.040;r=0.403,P=0.043;r=0.445,P=0.038).HAMD-17 量表当中的睡眠障碍因子与波幅绝对平均值、波幅标准方差、全频段总功率、α功率及δ功率呈显著正相关(r=0.451,P=0.016;r=0.431,P=0.028;r=0.437,P=0.021;r=0.451,P=0.045;r=0.456,P=0.023).抑郁组患者和不伴抑郁合并症状组患者波幅绝对值与波幅标准方差的差异有统计学意义(P<0.05).结论 颞叶癫痫患者非发作期间,脑电功率谱的波动增强与抑郁、睡眠问题密切相关,即波动性越明显,患者出现抑郁症状的概率越高.
Abstract
Objective To analyze the electrophysiological signals of temporal lobe epilepsy patients dur-ing the interval between seizures and explore their correlation with depressive symptoms.Methods Twenty hun-dred epilepsy patients who underwent surgical treatment in the Neurology Department of the Second People's Hospital of Zhengzhou from January 2022 to June 2023 were selected.According to the Hamilton Depression Scale,it is divided into a depression group and a non-depression group.By using stereotactic electroencephalogra-phy(EEG)and the Hamilton Depression Scale(HAMD-17),Pearson correlation analysis was used to investigate the characteristics of EEG signals during the intermittent period of temporal lobe epilepsy and their correlation with depressive symptoms.Result The total score of the HAMD-17 scale was significantly correlated with the absolute mean amplitude,standard deviation of amplitude,total power in the entire frequency band,θ power,and δ power of the anterior temporal gyrus(r=0.503,P=0.013;r=0.492,P=0.014;r=0.423,P=0.040;r=0.403,P=0.043;r=0.445,P=0.038).The sleep disorder factors in the HAMD-17 scale are significantly positively correlated with the absolute mean amplitude,standard deviation amplitude,total power of the entire frequency band,α power,and δ power(r=0.451,P=0.016;r=0.431,P=0.028;r=0.437,P=0.021;r=0.451,P=0.045;r=0.456,P=0.023).There was a statistically significant difference(P<0.05)in the absolute value and standard deviation of amplitude between pa-tients with depression and those without depression and accompanying symptoms.Conclusion During non-seizure periods,the increased fluctuation of EEG power spectrum in temporal lobe epilepsy patients is closely re-lated to depression and sleep problems.The more significant the fluctuation,the higher the probability of patients experiencing depressive symptoms.
