Objective To evaluate the accuracy of Sysmex XN-9000 body fluid mode classification for cerebrospinal fluid(CSF)nucleated cells.Methods Three hundred and fifty-six CSF samples from the clinical laboratory of Fujian Provincial Hospital from August 2022 to May 2023 were selected for both Sysmex XN-9000 body fluid mode detection and manual microscopy,and the percentage of nucleated cells was recorded.Passing and Bablock regression and Bland Altman plot were used to evaluate the consistency of instrument and manual microscopy results.Results The consistency between the instrument and manual microscopy results of the LY-BF%and HF-BF%in the group with nucleated cells in CSF(0~15)×106/μl was low(linear equation Y=21.640+0.800XY,Y=0.000+0.250X).The consistency of the NE-BF%,LY-BF%,and MO-BF%in the group with nucleated cells(16~30)×106/μl was high(linear equation Y=-1.016+1.000X,Y=2.933+0.967X,Y=0.595+1.038X).The consistency of the NE-BF%,LY-BF%,and MO-BF%in the group with nucleated cells>30×106/μl was high(linear equation Y=-0.433+1.000X,Y=2.296+0.954X,Y=1.783+0.943X).Conclusion CSF with a nuclear cell count of(0~15)× 106/μl should not be classified or undergone manual microscopy depending on clinical needs.When the instrument detects a very low proportion of EO-BF%and HF-BF%in a CSF sample with nucleated cell count higher than 15×106/μl,it can be directly reported,based on the instrument results.
