摘要
目的 分析探究处方前置审核系统对门诊处方合格率的影响.方法 采用回顾性分析方法,统计我院2023年1月至2023年10月门诊处方共计22 727张,自2023年6月起本院门诊启用了处方前置审核系统,启用前(2023年1月至5月)门诊处方共计11 760张,启用后(2023年6月至10月)门诊处方共计10 967张.对比处方前置审核系统启用前后门诊处方合格率、不合格处方分布情况,分析门诊处方前置审核系统的实施效果.结果 自2023年6月处方前置审核系统启用后,门诊处方总合格率为99.27％(10 887/10 967),处方前置审核系统启用前,门诊处方总合格率为95.88％(11 287/11 760),任用前的差异具有统计学意义(x2=333.713,P＜0.001).启用前门诊不合格处方共计473张(4.02％);启用后门诊不合格处方共计80张(0.73％),门诊处方不合格常见原因有适应证不适宜、用法用量不适宜、给药途径不适宜、联合用药不适宜、重复给药、内容缺项、书写不规范、超疗程用药等.处方前置审核系统启用后各种类型处方不合格率均有明显下降,其中适应证不适宜、用法用量不适宜、超疗程用药分别由1.27％、0.69％、0.77％大幅下降至0.18％、0.09％、0.08％.结论 处方前置审核系统通过智能审核和药师审核的双重机制,显著提高了药师的专业技能和服务质量,确保了用药的合理性、安全性和有效性,有效提高了处方质量,使门诊处方合格率显著提高,具有较高的应用价值.
Abstract
Objective To analyze and explore the impact of prescription pre-review system on the qualification rate of outpatient prescriptions.Methods A retrospective analysis was used to collect a total of 22727 outpatient prescriptions in our hospital from January 2023 to October 2023.Since June 2023,our outpatient department has implemented a prescription pre-review system,with a total of 11 760 outpatient prescriptions before use(to May 2023)and 10 967 outpatient prescriptions after use(June to October 2023).We compare the qualified rate and distribution of unqualified prescriptions in outpatient clinics before and after the implementation of the prescription pre-review system,and analyze the implementation effect of the outpatient prescription pre-review system.Results Since the launch of the prescription pre-review system in June 2023,the total qualified rate of outpatient prescriptions has been 99.26％.Before the launch of the prescription pre-review system,the total qualified rate of outpatient prescriptions was 95.24％,with a statistically significant difference(x2=333.71 3,P＜0.001).Before use,there were a total of 473 unqualified outpatient prescriptions,accounting for 4.02％.After use,there were a total of 80 unqualified outpatient prescriptions,accounting for 0.73％.The common reasons for unqualified outpatient prescriptions include inappropriate indications,inappropriate usage and dosage,inappropriate administration routes,inappropriate combination therapy,repeated administration,missing items in the content,non-standard writing,and exceeding the course of treatment.After the implementation of the prescription pre-review system,the failure rate of various types of prescriptions has significantly decreased.Among them,the indications are not suitable,the usage and dosage are not suitable,and the use of drugs beyond the treatment period has decreased significantly from 1.27％,0.69％,and 0.77％to 0.18％,0.09％,and 0.08％,respectively.Conclusion The prescription pre-review system significantly improves the professional skills and service quality of pharmacists through a dual mechanism of intelligent review and pharmacist's review,ensuring the rationality,safety,and effectiveness of medication,effectively improving prescription quality,significantly increasing the qualification rate of outpatient prescriptions,and has high application value.
