Objective To explore and summarize the clinical manifestations and ultrasound features of endometrial clear cell carcinoma. Methods A retrospective analysis was conducted on the clinical data and ultrasound images of 5 patients with endometrial clear cell carcinoma confirmed by surgical pathology in our hospital from January 2015 to January 2024.The clinical and ultrasound characteristics were analyzed and summarized. Results The average age of onset for the 10 cases of ECCC was(65.2±2.3)years old.The average maximum diameter of the lesion is(6.5±1.0)cm.Four cases(40%)were treated for palpation of abdominal masses, and six cases(60%)were treated for irregular vaginal bleeding.Eight cases of CA125 were all normal, while two case had mild elevation of CA125.Among the 10 cases of ECCC,6 cases(60%)showed solid echogenicity in a single chamber cyst, and 4 cases(40%)showed solid, uniform, and moderate echogenicity;4 cases had regular morphology, and 6 cases(60%)had irregular morphology;4 cases(40%)had clear boundaries with the muscular layer, while 6 cases(60%)had unclear boundaries with the muscular layer.Color Doppler showed a small amount of blood flow signal in 8 cases(80%),with resistance index(RI)all less than 0.4,and no blood flow signal in 2 cases(20%).FIGO staging:4 cases in stage Ⅰ,2 cases limited to the endometrium, and 2 cases invaded the superficial layer of the uterus;4 cases of stage Ⅱ,with lesions invading the cervical interstitium;2 cases in stageⅢ showed pelvic lymph node metastasis. Conclusion Endometrial clear cell carcinoma is more common in elderly women, and ultrasound findings have certain characteristics. The solid components in the cystic solid mass are irregular in shape, with uniform and moderate echogenicity. The cystic components are filled with dense punctate echogenicity, with ground glass like appearance; The solid mass has an irregular shape and presents a uniform moderate echogenicity.Combining clinical medical history can improve the preoperative diagnostic rate.
