摘要
探讨可视化椎间孔成形技术在腰椎间盘突出症(LDH)椎间孔镜手术治疗中的应用价值.方法 将我院2021年1月至2023年12月收治的76例LDH患者按手术方式分组.对照组38例为行单纯PTED治疗,观察组38例为在PTED加用可视化椎间孔成形技术.比较2组手术相关指标、腰椎功能、疼痛情况、腰功能障碍及手术安全性.结果2组出血量相当,但观察组手术耗时(53±11)min、椎间孔成形时间(23±6)min,短于对照组(74±13)min、(40±9)min,摄片次数(7.2±1.5)次少于对照组(12.9±2.7)次,并发症少于对照组,差异均有统计学意义(P＜0.05).2组术前腰椎功能、疼痛评分、肌力、提物、下肢功能、生活自理、疼痛以及总评分对比,差异无统计学意义(P＞0.05);2组术后腰椎功能评分均升高,疼痛评分、肌力、提物、下肢功能、生活自理、疼痛以及总评分均降低,且观察组变化更明显,差异有统计学意义(P＜0.05).结论LDH椎间孔镜手术治疗中采用可视化椎间孔成形技术效果较好,术中操作时间短、摄片次数少,术后疼痛轻,并发症发生率低,腰椎功能恢复效果更理想,值得推广应用.
Abstract
Objective To explore the application value of visualized intervertebral foraminoplasty technique in the treatment of lumbar disc herniation(LDH)with intervertebral foramen endoscopy surgery.Methods A total 76 LDH patients admitted to our hospital from January 2021 to December 2023 were divided into groups according the surgery.Thirty-eight cases in the control group were treated with simple PTED,while 38 cases in the observation group were treated with visualized intervertebral foraminoplasty technique in addition to PTED.We compared two groups of surgical related indicators,lumbar spine function,pain,lumbar dysfunction,and surgical safety.The results showed that the amount of bleeding in the two groups was equivalent,but the observation group had a shorter surgical time of(53±11)min and a shorter intervertebral foramen formation time of(23±6)min compared to the control group(74±13)min and(40±9)min.The number of films taken(7.2±1.5)was less than that of the control group(12.9±2.7),and the number of complications was less than that of the control group,with statistical significance(P＜0.05).There were no statistically significant differences in preoperative lumbar spine function,pain score,muscle strength,lifting,lower limb function,self-care,pain,and overall score between the two groups(P＞0.05).The postoperative lumbar spine function scores of both groups increased,while pain scores,muscle strength,lifting,lower limb function,self-care,pain,and overall scores decreased.The observation group showed more significant changes,with statistical differences(P＜0.05).Conclusion The use of visualized intervertebral foraminoplasty technique in the treatment of LDH with intervertebral foramen endoscopic surgery has a good effect,with short operation time,fewer imaging times,mild postoperative pain,low incidence of complications,and more ideal recovery of lumbar function.It is worth promoting and applying.
