Objective To analyze thyroid function, blood lipid and pregnancy outcome in patients with hypothyroidism and subclinical hypothyroidism. Methods A total of 84 patients with gestational hypothyroidism admitted to our hospital from January 2021 to January 2023 were selected in the study.Among them,35 patients with gestational hypothyroidism were classified as Group A,49 patients with gestational subclinical hypothyroidism were classified as Group B.In addition,45 cases with normal thyroid function were selected from pregnant women who gave birth in our hospital as group C.According to the maternal and infant outcomes of pregnant women with hypothyroidism, they were divided into a negative maternal and infant outcome group of 68 cases(81.0%)and a normal maternal and infant outcome group of 16 cases(19.1%).Thyroid function, serum homocysteine(Hcy)level and blood lipid level were compared among the three groups, and the occurrence of adverse maternal and infant outcomes in the three groups was recorded, and the levels of serum thyrotropin(TSH)and Hcy in patients with different maternal and infant outcomes were compared. Results The levels of TSH and Hcy in group A were significantly higher than those in groups B and C,and the levels of free thyroxine(FT4)and free triiodothyronine(FT3)were lower than those in groups B and C(P<0.05).The levels of triglycerides(TG)and total cholesterol(TC)in group A were higher than those in group B and C,while the levels of high-density lipoprotein cholesterol(HDL-C)were lower than those in group B and C(P<0.05);The total incidence of adverse maternal and infant outcomes in Group A and Group B was higher than that in Group C(x2=54.869,P<0.001);The levels of TSH and Hcy in patients with adverse maternal infant outcomes of pregnancy combined with hypothyroidism were higher than those in the normal maternal infant outcome group(t=8.248,t=4.653,P<0.001).Conclusion Hypothyroidism in pregnancy is closely related to maternal and infant outcomes. The increase of TSH and Hcy levels will lead to an increase in the incidence of adverse maternal and infant outcomes, and hypothyroidism will affect the blood lipid level of patients.
