摘要
目的:观察不同剂量盐酸戊乙奎醚用于小儿全身麻醉手术时对小儿心率、体温及腺体分泌的影响.方法:100例患儿随机分为5组各20例,Ⅰ组为空白对照组,Ⅱ组为阿托品组,Ⅲ组为盐酸戊乙奎醚0.010 mg/kg组,Ⅳ组为盐酸戊乙奎醚0.015 mg/kg组,Ⅴ组为盐酸戊乙奎醚0.030 mg/kg组,分别于麻醉诱导前即刻,用药前、用药后10、20、30min及术毕时记录心率、体温及唾液分泌量.结果:与给药前比较,Ⅰ组术毕唾液分泌量明显增多;与Ⅰ组比较,Ⅱ、Ⅲ、Ⅳ组唾液分泌量在术毕均明显减少,Ⅳ组给药后10、20、30 min及术毕唾液分泌量均明显减少.与给药前比较,Ⅲ组给药后10、20、30 min及术毕心率均明显下降;与Ⅰ组比较,Ⅱ组给药后20、30 min心率明显增快,Ⅲ组术毕心率明显下降.体温除Ⅴ组术毕较给药前明显下降外,余各组差异均无统计学意义.结论:小儿全身麻醉手术选择O.03 mg/kg盐酸戊乙奎醚作为抗胆碱能药物,能有效抑制腺体分泌,保持术中心率稳定.
Abstract
Objective To evaluate the effect of different dosages of penehyclidine hydrochloride on the heart rate, temperature and salivation volume in pediatric general anesthesia. Methods One hundred patients were randomly divided into 5 groups, group Ⅰ receiving placebo, group Ⅱ receiving atropine 0.01 mg/kg, group Ⅲ receiving penehyclidine hydrochloride 0.01 mg/kg, group Ⅳ receiving 0. 015 mg/kg, and group Ⅴ receiving 0. 03 mg/kg, 20 patients each. The heart rate, body temperature and salivation volume were recorded before medication, 10 minutes, 20 minutes, 30 minutes after medication and at the end of operation. Results The salivation volume in group Ⅰ increased significantly at the end of operation compared with that before medication. Compared with group Ⅰ , the salivation volume at the end of operation decreased significantly in group Ⅱ , Ⅲ and Ⅳ,and decreased significantly after medication in group Ⅴ. The heart rate in group Ⅲ decreased significantly after medication, and compared with that in group Ⅰ before medication, it increased significantly 20 and 30 minutes after medication in group Ⅱ , and decreased significantly at the end of operation in group Ⅲ. The temperature was lower obviously after operation compared with that before medication in group Ⅴ, and it didn't change significantly in the other four groups. Conclusion Penehyclidine hydrochloride as an anticholinergic agent at the dose 0.03 mg/kg can effectively inhibit the secretion and keep a stable heart rate in pediatric general anesthesia.
