中医辨证论治结合俯卧位通气治疗ARDS的疗效观察

摘要
目的 分析中医辨证论治结合俯卧位通气治疗急性呼吸窘迫综合症(ARDS)的临床疗效.方法 选取 2022 年 7月至2023 年9 月在我院呼吸内科与两个监护病区收治的第一诊断为ARDS患者126 例,其中监护一病区的42 例患者分为俯卧位通气组,监护二病区的42 例分为中药治疗组,呼吸内科病区42 例分为联合治疗组.俯卧位通气组在常规治疗基础上给予俯卧位通气治疗,中药治疗组在常规治疗基础上给予中医辨证论治后口服中药治疗,联合治疗组在常规治疗基础上给予俯卧位通气联合中医辨证论治后口服中药治疗,观察比较3 组患者治疗7d后的炎症指标、动脉血气分析指标及肺损伤生物学标志物.结果 俯卧位通气组与中药治疗组的炎症指标、动脉血气指标、肺损伤生物学标志物指标比较,差异均无统计学意义(均P>0.05);联合治疗组的炎症指标、动脉血气指标、肺损伤生物学标志物指标改善均优于中药治疗组和俯卧位通气组,差异均有统计学意义(均P<0.05).结论 中医辨证论治结合俯卧位通气治疗ARDS能显著降低相关炎症指标与肺损伤生物学标志物,改善动脉血气分析指标,提高治疗效果.
Abstract
Objective To analyze the clinical efficacy of traditional Chinese medicine(TCM)combined with prone ventilation in the treatment of ARDS.Methods A total of 126 patients with the first diagnosis of ARDS admitted to our hospital from July 2022 to September 2023 were selected,of which 42 patients in the first ward were in the prone ventilation group and 42 patients in the second ward were divided into TMC treatment group.42 patients in the respiratory medicine ward were divided into the combined treatment group.The prone ventilation group was given prone ventilation treatment on the basis of conventional treatment,the TCM treatment group was given oral TCM on the basis of conventional treatment,and the combination treatment group was given TCM combined with prone ventilation treatment on the basis of conventional treatment.The inflammatory indexes,arterial blood gas indexes and lung injury biomar-kers of the three groups were observed and compared after 7 days of treatment.Results After treatment,there was no significant difference in inflammatory indexes,arterial blood gas indexes and biomarkers of lung injury between the prone ventilation group and the TMC treatment group(all P>0.05).The indexes of inflammation,arterial blood gas and biomarkers of lung injury in combined treat-ment group were better than those in TCM treatment group and prone position ventilation group,and the differences were statistically sig-nificant(all P<0.05).Conclusion The combination of TCM and prone ventilation in the treatment of ARDS can significantly re-duce the related inflammatory markers and biomarkers of lung injury,improve the arterial blood gas analysis indexes,and improve the treatment effect.
