静脉全麻下全方位手法松解联合中药溻渍治疗原发性冻结肩的疗效观察

摘要
目的 观察静脉全麻下全方位手法松解联合中药溻渍治疗原发性冻结肩(PFS)的疗效.方法 选取2022 年2 月至2023 年4 月我院收治的85 例PFS患者为研究对象,按随机数字表法分为对照组(n =43)与研究组(n =42).对照组给予静脉全麻下全方位手法松解治疗,研究组在对照组的基础上给予中药溻渍治疗,比较两组患者的临床疗效、疼痛程度[视觉模拟评分法(VAS)]、肩关节活动度,并采用加州大学肩关节评分系统(UCLA)评定两组患者的肩关节功能.结果 研究组的总有效率较对照组高,差异有统计学意义(P<0.05).研究组经治疗1、2、3 个月时的VAS评分均低于对照组,差异均有统计学意义(均P<0.05).研究组治疗3 个月时的肩关节外展度、内旋度、外旋度以及UCLA评分均较对照组高,差异均有统计学意义(均P<0.05).结论 静脉全麻下全方位手法松解联合中药溻渍治疗PFS的效果好,可减轻疼痛程度,提高肩关节活动度,改善肩关节功能.
Abstract
Objective To observe the curative effect of comprehensive manual release under intravenous general anesthesia com-bined with traditional Chinese medicine soak in the treatment of primary frozen shoulder(PFS).Methods 85 patients with PFS ad-mitted to our hospital from February 2022 to April 2023 were selected as the study objects,and were divided into control group(n =43)and research group(n =42)according to random number table method.The control group was given comprehensive manual release treatment under intravenous general anesthesia,and the research group was given Chinese medicine soak on the basis of the control group.The efficacy,pain degree,[visual analogue scale(VAS)]and shoulder motion of the two groups were compared,and the shoul-der joint function of the two groups was assessed by the Shoulder Joint Scoring System(UCLA)of California University.Results The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).The VAS scores of the study group at 1,2 and 3 months after treatment were lower than those of the control group,with statistical significance(all P<0.05).After3 months of treatment,shoulder extension,internal rotation,external rotation and UCLA score of the study group were higher than those of the control group,with statistical significance(all P<0.05).Conclusion Comprehensive manual release under intravenous general anesthesia combined with soak is effective in the treatment of PFS,which can reduce pain,im-prove shoulder joint motion and improve shoulder joint function.
