摘要

目的 探讨支气管肺泡灌洗(BAL)治疗重症肺部感染的临床效果.方法 选取2022 年10 月至2023 年7 月我院收治的重症肺部感染患者121 例作为研究对象,采取随机数字表法分为对照组(n=60)和观察组(n=61).对照组给予常规治疗,观察组在对照组基础上加用BAL治疗.比较两组患者的临床疗效以及治疗前后的气道阻力(RAW)、呼吸做功(WOB)、气道峰值吸气压力(PIP)、血氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)水平.结果 观察组的总有效率为93.44%,高于对照组的70.00%,差异有统计学意义(P<0.05).观察组治疗后的RAW、WOB、PIP水平均低于对照组,差异均有统计学意义(均P<0.05).观察组治疗后PaO2、PaO2/FiO2 水平均明显高于对照组,PaCO2 水平低于对照组,差异均有统计学意义(均P<0.05).结论 BAL治疗重症肺部感染患者的疗效显著,可改善患者呼吸功能.

Abstract

Objective To investigate the clinical effect of broncho alveolar lavage(BAL)in the treatment of patients with severe pulmonary infection.Methods 121 patients with severe pulmonary infection admitted to our hospital from October 2022 to July 2023 were selected,who were divided into control group(60 cases)and observation group(61 cases)by the random number table method.The control group adopted conventional treatment,and observation group was given BAL based on the control group.The clinical effica-cy,airway resistance(RAW),respiratory work(WOB),airway inspiratory pressure(PIP),blood oxygen partial pressure(PaO2),crbon dioxide partial pressure(PaCO2),oxygenation index(PaO2/FiO2)of two groups were compared before and after treatment.Results The total effective rate in the observation group was higher than that in the control group(93.44%vs 70.00%),the differ-ence was statistically significant(P<0.05).RAW,WOB,PIP of observation group were lower than that in the control group after treat-ment,the differences were statistically significant(all P<0.05).PaO2 and PaO2/FiO2 in observation group were significantly higher than that of control group,and PaCO2 was significantly lower than that of the control group,the differences were statistically significant(all P<0.05).Conclusion BAL is effective in treating patients with severe pulmonary infection,which can improve their respira-tory function.
