血清CD36水平及CD36rs1049673基因多态性与动脉粥样硬化性脑梗死的相关性研究

摘要
目的 研究血清CD36 水平及CD36rs1049673 位点基因型与动脉粥样硬化性脑梗死的相关性.方法 选择 2020年7 月至2023 年10 月我院诊断为动脉粥样硬化性脑梗死患者92 例作为脑梗死组,同时选择健康体检者 41 例作为对照组,检测并比较两组患者的总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、空腹血糖(FBS)及血清CD36 水平.采用荧光多重酶连接反应(iMLDR)技术分析CD36rs1049673 位点等位基因和基因型,比较两组患者基因型与等位基因频率.采用Logistic回归模型分析血清CD36 水平及CD36rs1049673 基因型与脑梗死的相关性.结果 脑梗死组患者的TG、TC、LDL-C、FBS、血清CD 36 水平显著高于对照组,HDL-C低于对照组,差异均有统计学意义(均P<0.05).CD36rs1049673 位点基因型包括GG、GC、CC,等位基因包括G和C.两组CD36rs1049673 位点基因型与等位基因的分布频率比较,差异均有统计学意义(χ2=7.327、4.444,P=0.026、0.035).显性模型GG基因型与GC+CC基因型在脑梗死组与对照组中的分布频率比较,差异有统计学意义(χ2=7.288,P=0.007).GG基因型与GC基因型比较,GG基因型能增加脑梗死发病的风险.Logistic多元逐步回归分析结果显示,TG(OR=3.363,P=0.001)、血清CD36 水平升高(OR=1.098,P=0.023)和CD36rs1049673GG、GC+CC基因型(OR=3.521,P=0.029)是脑梗死发病的独立危险因素,HDL-C水平升高(OR=0.012,P≤0.001)起保护作用.结论 TG、血清CD36 水平升高和CD36rs1049673 GG基因型是脑梗死发病的独立危险因素,HDL-C水平升高起保护作用,研究结果可为动脉粥样硬化性脑梗死的干预和诊断提供指导.
Abstract
Objective To study the relationship between serum CD36 level and CD36rs1049673 genotype and atherosclerotic cere-bral infarction.Methods 92 patients with atherosclerotic cerebral infarction(cerebral infarction group)and 41 healthy subjects(control group)were selected from our hospital from July 2020 to October 2023.Total cholesterol(TC),high density lipoprotein cho-lesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),triglyceride(TG),fasting blood glucose(FBS)and serum CD36 lev-els were detected and compared between the two groups.The alleles and genotypes of CD36rs1049673 were analyzed by fluorescence multiple enzymes linking reaction(iMLDR)technique,and the genotype and allele frequencies of the two groups were compared.Logis-tic regression model was used to analyze the correlation between serum CD36 level and CD36rs1049673 genotype and cerebral infarc-tion.Results The levels of TG,TC,LDL-C,FBS and serum CD36 in cerebral infarction group were significantly higher than those in control group,while HDL-C was lower than that in control group(P<0.05).The genotype of CD36rs1049673 includes GG,GC and CC,and the alleles include G and C.There were significant differences in genotype and allele distribution frequency of CD36rs1049673 between the two groups(χ2=7.327,4.444,P=0.026).The distribution frequency of GG genotype and GC+CC genotype in the dom-inant model was significantly different between the cerebral infarction group and the control group(χ2=7.288,P=0.007).GG geno-type increased the risk of cerebral infarction compared with GC genotype.Logistic multiple stepwise regression analysis showed that TG(OR=3.363,P=0.001),serum CD36 level increased(OR=1.098,P=0.023)and CD36rs1049673 GGvs GC+CCgenotype(OR=3.521,P=0.029)was an independent risk factor for cerebral infarction,and increased HDL-C level(OR=0.012,P=<0.001)had a protective effect.Conclusion Logistic multiple stepwise regression analysis showed that TG,elevated serum CD36 levels and CD36rs1049673 GG genotype were independent risk factors for cerebral infarction,and elevated HDL-C levels played a protective role,which may provide guidance for the intervention and diagnosis of atherosclerotic cerebral infarction.
