慢性阻塞性肺病稳定期患者外周血Treg细胞与细胞毒性T淋巴细胞相关抗原-4的表达及意义

摘要
目的 检测吸烟者及慢性阻塞性肺疾病(COPD)患者外周血细胞毒性T淋巴细胞相关抗原-4(CTLA4)/B7 及Treg细胞的表达,观察免疫指标在COPD外周血中的变化及临床意义.方法 选择8 名健康志愿者为A组(正常对照组)、8 名吸烟但肺功能正常者为B组、8 名轻中度慢性阻塞性肺疾病患者为C组,收集3 组研究对象的外周血标本,采用流式细胞术分析方法检测各组外周血单个核细胞 CD11c+树突状细胞(DCs)的比例、CD11c+DCs 表面 CD80 及 CD86 的表达,Treg 细胞(CD4+CD25+FOXP3+)及CD4+CD25+CTLA4+的表达.结果 与 A组比较,B 组及 C 组的 CD11+DCs、CD80(B7-1)和CD86(B7-2)表达均显著增多,差异均有统计学意义(均P<0.05).在Treg细胞表达上,与A组相比,B组及C组的Treg细胞减少(均P<0.05),而B组与C组间比较无统计学意义(P>0.05).与A组相比,B组及C组的CTLA4+细胞表达增多,差异均有统计学意义(均P<0.05).结论 吸烟者及COPD患者外周血中CTLA4/B7 呈高表达,Treg细胞比例减少,提示吸烟所致免疫细胞异常可能参与COPD全身炎症进展过程.
Abstract
Objective To investigate the expression of cytotoxic T lymphocyte-associated antigen-4(CTLA4)/B7 and Treg cells in the peripheral blood of smokers and patients with chronic obstructive pulmonary disease(COPD),and to examine the changes and clin-ical significance of immune indicators in COPD.Methods This study recruited 8 healthy volunteers as the normal control group(Group A),8 smokers with normal lung function(Group B),and 8 patients with mild to moderate COPD(Group C).Flow cytometry analysis was utilized to assess the proportion of CD11c+dendritic cells(DCs)in peripheral blood mononuclear cells,the surface ex-pression of CD80 and CD86 on CD11c+DCs,as well as Treg cells(CD4+CD25+FOXP3+)and CD4+CD25+CTLA4+cells.Re-sults In comparison to group A,the expression of CD11+DCs,CD80(B7-1),and CD86(B7-2)significantly increased in groups B and C,with statistically significant differences(all P<0.05).Regarding Treg cells,the number decreased in groups B and C compared to group A,with statistically significant differences(all P<0.05),but no statistical variance between groups B and C(P>0.05).Ad-ditionally,the expression of CTLA4+cells in groups B and C increased significantly compared to group A(all P<0.05).Conclusion High expression of CTLA4/B7 and reduced Treg cell proportions in the peripheral blood of smokers and COPD patients suggest a po-tential link between smoking-induced immune cell abnormalities and systemic inflammation in COPD.
