摘要

目的 探究床旁超声在新生儿脑病(NE)中的应用价值.方法 选择2020 年8 月至2022 年7 月在我院就诊的疑似NE患儿88 例的临床资料,所有患儿均行床旁超声及MRI扫描检查.分析床旁超声与MRI检查结果,对比NE患儿与未发生NE患儿大脑中动脉(MCA)血流参数.结果 相比于MRI检查,床旁超声在脉络丛囊肿、Ⅰ级颅内出血、室管膜下囊肿中的检出率均较高,但蛛网膜及硬脑膜下腔出血的检出率较低,差异均有统计学意义(均P<0.05).将彩超检出的NE患儿74 例纳入NE组,未发生NE患儿14 例纳入无NE组,两组患儿的收缩期血流速度(Vs)、舒张期血流速度(Vd)、阻力指数(RI)、收缩期峰值流速/舒张末期流速(S/D)水平比较,NE组患儿的各指标水平均高于无NE组,差异均有统计学意义(均P<0.05).结论 床旁超声及MRI检查均可用于NE的鉴别诊断,床旁超声检查在提升脉络丛囊肿、室管膜下囊肿等疾病类型的检出率优于MRI,且具有操作便捷、无创性、重复性高等优势,可为临床诊疗提供可靠的参考依据.

Abstract

Objective To explore the application value of bedside ultrasound in neonatal encephalopathy(NE).Methods Clinical data of 88 children with suspected NE admitted to our hospital from August 2020 to July 2022 were selected,all of whom were examined by bedside ultrasound and magnetic resonance imaging(MRI).The results of bedside ultrasound and MRI were analyzed,and the blood flow parameters of middle cerebral artery(MCA)in children with NE and children without NE were compared.Results Compared with MRI,the detection rates of bedside ultrasound in choroid plexus cyst,grade Iintracranial hemorrhage and subependymal cyst were higher,but the detection rates of arachnoid and subdural hemorrhage were lower,and the differences were statistically signifi-cant(all P<0.05).Seventy-four children with NE detected by color ultrasound were included in the NE group,and 14 children without NE were included in the NE group.The systolic blood flow velocity(Vs),diastolic blood flow velocity(Vd),resistance index(RI)and peak systolic flow velocity/end-diastolic flow velocity(S/D)of the two groups were compared,and the levels of all indicators in the NE group were higher than those in the NE group.The differences were statistically significant(all P<0.05).Conclusion Both bed-side ultrasound and MRI can be used in the differential diagnosis of NE.Compared with MRI,bedside ultrasound can significantly im-prove the detection rate of choroidal cysts,subependymal cysts and other diseases in children with NE,and has the advantages of con-venient operation,non-invasive and high repeatability,which can provide a reliable reference for clinical diagnosis and treatment.
