摘要
目的 观察泻火解毒法治疗原矛头蝮蛇咬伤的临床疗效.方法 选择2022年7月1日至2023年8月1日在我院治疗的原矛头蝮蛇咬伤患者108例,按随机数字表法分为治疗组和对照组各54例.对照组给予常规治疗,治疗组在对照组的基础上给予口服蛇伤胶囊(泻火解毒法治疗毒蛇咬伤代表方)治疗,观察两组患者治疗后第1、3、5、7天时的患肢周径差、肿胀消退时间、治疗疗程及患肢疼痛评分.结果 两组患者治疗后第1、3天时的患肢周径差比较,差异均无统计学意义(均P＞0.05);治疗后第5、7天时,治疗组的患肢周径差明显低于对照组,差异均有统计学意义(均P＜0.05).治疗组的患肢肿胀消退时间、治疗疗程均短于对照组,差异有统计学意义(均P＜0.05).治疗第3、5、7天时,治疗组的患肢疼痛评分均明显低于对照组,差异均有统计学意义(均P＜0.05).结论 泻火解毒法治疗原矛头蝮蛇咬伤的临床疗效好,值得推广应用.
Abstract
Objective To observe the clinical efficacy of the method of purging fire and detoxifying in the treatment of patients bit-ten by Protobothrops mucrosquamatus.Methods A total of 108 patients with original spearhea Protobothrops mucrosquamatus bite who were treated from July 1,2022 to August 1,2023 were randomly divided into a treatment group and a control group,with 54 patients in each group.The control group was treated with snake bite scar incision for decompression and detoxification,antivenom serum to neu-tralize free snakes,antibiotics to resist infection,glucocorticoids to anti-inflammatory,tetanus antitoxin to prevent tetanus,and the rest received symptomatic treatment;On the basis of the above diagnosis and treatment plan,the treatment group was treated with oral snake wound capsules.The peripheral diameter differences of the affected limbs of the two groups of patients on days 1,3,5,and 7 were ob-served,and the time for swelling to subside,treatment course,and pain score of the affected limbs were compared between the two groups.Results There was no significant difference in limb circumference difference between the treatment group and the control group on the first and third day of treatment(all P＞0.05),and there was no statistical significance;On the 5th and 7th day of treat-ment,the circumferential diameter difference in the treatment group was lower than that in the control group,with statistical significance(all P＜0.05);The time for the swelling of the affected limbs to subside and the treatment course in the treatment group were signifi-cantly shorter than those in the control group(all P＜0.05),with statistical significance.The pain scores of the two groups of patients were statistically significant compared between the groups on the 3rd,5th,and 7th day of treatment(all P＜0.05).Conclusion The method of purging fire and detoxifying is clinically effective in treating the bite of the original spear head Protobothrops mucrosquamatus,and is worth promoting and applying.
