摘要

目的 分析急性心肌梗死患者急诊术前院内流程现状,并绘制价值流程图(VSM),为院内流程改善提供依据,以提高患者救治效率.方法 选择2022年3月至2023年3月在我院救治的93例急性心肌梗死患者为研究对象.小组成员记录患者抵达急诊分诊台到被送入手术室或介入中心的每一个院内流程环节的时间.基于VSM的标准图标和已记录的时间信息,绘制急性心肌梗死患者急诊术前院内流程VSM.结果 急性心肌梗死患者急诊术前院内工作时间(P/T)为14~71 min,等待时间(W/T)为6~262 min,总流程时间(L/T)为20~333 min,其中家属缴费、心电图检查、抽血送检、术前准备、家属参与决策、办理入院和患者入手术室或介入中心的等待时间较长.结论 急性心肌梗死患者急诊术前院内流程仍有较大的优化空间,进一步优化急诊术前院内流程形成精细化管理,以提高医疗服务质量和急性心肌梗死的救治效率.

Abstract

Objective To observe and analyze the current situation of emergency preoperative in-hospital processes in patients with acute myocardial infarction,and to draw a value stream mapping(VSM)to provide a basis for the improvement of in-hospital proces-ses,in order to improve the treatment efficiency of such patients.Methods A total of 93 patients with acute myocardial infarction who were treated in our hospital from March 2022 to March 2023 were enrolled.From the moment the patient arrives at the emergency triage table,team members begin the time-lapse until the patient is admitted to the operating room or interventional center,detailing the timing of each in-hospital process link.Based on the standard icons of VSMs and the recorded time information,we plotted the VSMs of the preoperative in-hospital flow in patients with acute myocardial infarction.Results For patients with acute myocardial infarction,the preoperative working time was 14-71 minutes,while the waiting time was 6-262 minutes,and the total process time was 20-333 mi-nutes.Conclusion At present,there is still a lot of space for optimization of the preoperative in-hospital process of patients with a-cute myocardial infarction,and it is necessary to further optimize it to achieve refined management and humanized services,and to opti-mize the in-hospital process of patients with acute myocardial infarction in the form of green channels,in order to improve the efficiency of treating patients with acute myocardial infarction.
