摘要
目的 研究CD36 多态性位点rs1761667 和rs1054517 及血栓弹力图各项指标与脑梗死的相关性.方法 选择2020 年7 月至2023 年11 月在我院治疗的93 例动脉粥样硬化性脑梗死患者为脑梗死组,选择同期在我院的健康体检者86 例为对照组.收集两组的性别、年龄、身高、体重、体质量指数(BMI)、收缩压和舒张压资料.采用荧光多重高温连接酶检测反应(iMLDR)技术分析CD36 rs1761667 和rs1054517 位点基因,采用血栓弹力图检测两组的凝血反应时间(R值)、血凝块形成时间(K值)、血凝块形成速率(α角)、血凝块形成最大振幅(MA值).结果 脑梗死组的身高、体重、BMI、收缩压、舒张压均高于对照组,α角和MA值均大于对照组,R值和K值小于对照组,差异均有统计学意义(均P<0.05).脑梗死组在CD36 位点的rs1054517 的T等位基因频率(56.99%)大于对照组(45.35%),CT基因型比CC基因型的R值缩短[(5.18±2.13)min VS(6.71±3.59)min],差异均有统计学意义(均P<0.05);TT基因型的R值短于CC基因型[(5.92±2.24)min VS(6.71±3.59)min],但未达到统计学差异(P>0.05).结论 脑梗死患者的BMI、收缩压和舒张压偏高以及血液高凝状态是其发病的主要原因,CD36 位点rs1054517 携带T等位基因或该位点CT杂合子基因型患者的R较短,其凝血反应时间缩短、血液高凝状态可能与脑梗死有直接相关性.
Abstract
Objective To study the relationship between CD36 polymorphic sites rs1761667 and rs1054517 and the indexes of thrombelastogram and cerebral infarction.Methods 93 patients with atherosclerotic cerebral infarction treated in our hospital from July 2020 to November 2023 were selected as the cerebral infarction group,and 86 healthy subjects in our hospital during the same period were selected as the control group.Gender,age,height,weight,body mass index(BMI),systolic and diastolic blood pressure were collected.The genotypes of CD36 rs1761667 and rs1054517 were analyzed by fluorescence multiple enzyme linking reaction(iMLDR)technique.The coagulation reaction time(R),hemagglutination time(K),coagulation Angle α,and maximum clot ampli-tude(MA)of all subjects were detected by thrombelastogram.Results The height,weight,BMI,systolic blood pressure and dias-tolic blood pressure of cerebral infarction group were higher than those of control group,α and MA values were higher than those of con-trol group,R and K were lower than those of control group,the differences were statistically significant(all P<0.05).The T allele frequency of rs1054517 at CD36 site in cerebral infarction group(56.99%)was higher than that in control group(45.35%),and the R value of CT genotype was shorter than that of CC genotype than that of control group(5.18±2.13 and 6.71±3.59),with statisti-cal significance(all P<0.05).The R value of TT genotype carriers was shorter than that of CC genotype carriers(5.92±2.24 and 6.71±3.59),but the difference was not statistically significant(P>0.05)Conclusion Obesity,high systolic and diastolic blood pressure,and hypercoagulability of the blood are the main causes of cerebral infarction in patients.Individuals who carry the T allele of the CD36 locus rs1054517 or have the CT heterozygous genotype have shorter R values,and their coagulation reaction time is shorter,which may be directly related to cerebral infarction..
