二甲基甲酰胺职业暴露与慢性非萎缩性胃炎的相关性研究

摘要
目的 探讨二甲基甲酰胺职业暴露与作业人员慢性非萎缩性胃炎的相关性.方法 对4家企业385名员工进行健康状况调查,将发现的28例慢性非萎缩性胃炎现患者设为病例组,所有对象经性别、年龄排序后,按性别一致、年龄±1岁分组,在每个病例对应的组中,统一依次按“隔位选一”原则选取4名无现患者(1∶4配对),共112名工人为对照组,开展病例对照研究.相关性分析与评价采用卡方检验和条件Logistic回归分析.结果 (1)慢性非萎缩性胃炎现患率在性别上基本保持一致,但在年龄分组上存在一定差异(P＜0.05)；(2)病例组二甲基甲酰胺接触率显著高于对照组,接触工龄≤5年者尤为突出(P＜0.05)；(3)调整吸烟、饮酒和其他职业病危害因素接触等混杂因素后,二甲基甲酰胺接触与患慢性非萎缩性胃炎的危险性增加显著相关,接触二甲基甲酰胺≤5年患慢性非萎缩性胃炎的危险较明显(P＜0.05).结论 二甲基甲酰胺职业暴露可增加患慢性非萎缩性胃炎的危险.
Abstract
Objective To study the association of dimethylformamide(DMF) exposure with chronic non-atrophic gastritis. Methods Firstly, a cross sectional survey was performed. 385 workers from 4 factories were examined, among them 28 cases of chronic non-atrophic gastritis were found as case group. All the objects were sorted according to gender and age. 1:4 match principle was used in the study that was 4 controls/1 case in each sub-group, therefore, 112 controls were selected. Additionally, Chi-square test and multivariable conditional logistic regression model were used for analysis and assessment of the association. Results (1) There was a similar prevalence of chronic non-atrophic gastritis in men and women; the prevalence was different with age(P<0.05). (2) The DMF exposure prevalence was significantly higher in case group. The major proportion of exposure duration were ≤5 years(P <0. 05) . (3) After adjusting the confounding factors such as smoking, drinking and other occupational exposure etc. the risk of chronic non-atrophic gastritis was obviously correlated to DMF exposure, especially the exposure less than 5 years (P <0. 05). Conclusion The results suggested that DMF exposure may increase the risk of chronic non-atrophic gastritis.
