Diagnostic value of serum SFRP5 level and LH/FSH ratio combined with ovarian ultrasound in patients with polycystic ovary syndrome-related infertility
Objective:To explore the diagnostic value of serum secreted frizzled-related protein 5(SFRP5)level,luteinizing hormone/follicle stimulating hormone(LH/FSH)ratio combined with ovarian ultrasound in patients with polycystic ovary syndrome(PCOS)-related infertility.Methods:A total of 135 patients with PCOS and infertility recruited in Ultrasound Diagnosis Department of Wuhan Traditional Chinese Medicine Hospital from January 2022 to June 2023 were assigned to the infertility group,another 130 healthy women who underwent prenatal examinations were collected as the control group.Color Doppler ultrasound was used to evaluate the number of follicles,maximum follicle diameter,ovarian pulsatile index(PI),and resistance index(RI).Pearson/Spearman correlation test was conducted to analyze the correlation of serum SFRP5 level as well as LH/FSH ratio with ultrasound parameters.A receiver operating characteristics(ROC)curve was applied to analyze the diagnostic value of serum SFRP5 level and LH/FSH ratio combined with ovarian ultrasound for PCOS-related infertility.Results:Compared with the control group,serum SFRP5 level[(13.68±3.25)ng/ml vs.(17.50±3.42)ng/ml],maximum follicle diameter[(8.67±1.75)mm vs.(11.83±2.88)mm],PI[(0.82±0.14)vs.(0.98±0.20)]and RI[(0.56±0.11)vs.(0.65±0.13)]were significantly decreased(P<0.05),while LH/FSH ratio[(2.13±0.45)vs.(1.49±0.31)],homeostasis model insulin resistance index(HOMA-IR)[(2.94±0.53)vs.(2.36±0.42)]and the number of follicles[(15.31±4.26)vs.(9.52±2.19)]were significantly increased in PCOS-related infertility group(P<0.05).Serum SFRP5 level was negatively correlated with the number of follicles(P<0.05),and positively correlated with maximum follicle diameter,PI and RI(P<0.05).Serum LH/FSH ratio was positively correlated with the number of follicles(P<0.05),and negatively correlated with maximum follicle diameter,PI and RI(P<0.05).Serum SFRP5 level,maximum follicle diameter,PI and RI were independent protective factors for PCOS-related infertility(OR=0.724,0.713,0.731,0.741,respectivly,P<0.05),while LH/FSH ratio,HOMA-IR,and the number of follicles were independent risk factors for PCOS-related infertility(OR=1.349,1.436,1.352,respectively,P<0.05).The area under ROC curves(AUCs)of serum SFRP5 level,LH/FSH ratio,the number of follicles,maximum follicle diameter,PI and RI for diagnosing PCOS-related infertility were 0.819,0.855,0.786,0.755,0.834,and 0.813,respectively,and the AUC of the combination of the six indicators for diagnosing PCOS-related infertility was 0.952(P<0.05).Conclusions:The serum SFRP5 level decreases and LH/FSH ratio increases in patients with PCOS-related infertility,and the combination of ovarian ultrasound parameters has high diagnostic value for PCOS-related infertility.
