Objective:To explore the clinical application effects of continuation care in self-health management among rural elderly patients with coronary heart disease.Methods:Rural coronary heart disease patients hospitalized in a Traditional Chinese Medicine Hospital in Tangshan from 2021 to 2023 were selected as the study subjects.They were randomly divided into an intervention group(101 patients)and a control group(104 patients).The intervention group received continuation care,while the control group received conventional health education.Differences in family function,physical activity,self-care ability,quality of life,and complications between the two groups were analyzed and compared after the intervention.Results:Before the intervention,there were no statistically significant differences in general information such as age,gender,BMI,and disease duration between the two groups(P>0.05).After the intervention,scores for family function,physical activity,self-care ability,and quality of life in both groups improved compared to before the intervention(P<0.05).The reduction in family function and physical activity scores was significantly greater in the continuation care group than in the conventional care group(P<0.05).The increase in scores for self-concept,self-care knowledge,and self-care skills in the intervention group was significantly higher than that in the control group(P<0.001).There was no statistically significant difference in the increase of self-care responsibility scores between the two groups(P>0.05).The increase in scores for mental health,emotional function,general health,social function,vitality,and bodily pain in the intervention group was significantly higher than that in the control group(P<0.001).The incidences of complications such as heart failure,arrhythmia,and angina pectoris were lower in the intervention group than in the control group.Conclusion:Continuation care is highly effective in health management for elderly patients with coronary heart disease in rural areas.
