交感神经对神经病理性痛自发痛的影响

自发痛(spontaneous pain)是在没有明显外界刺激诱发的情况下存在的疼痛,是神经病理性痛的主要临床表现之一,其发病机制复杂,目前缺乏有效的治疗手段,给神经病理性痛患者带来极大的困扰。研究发现,神经病理性痛自发痛与交感神经系统功能紊乱有关。临床上采用交感神经阻滞术或切除术对神经病理性痛自发痛有一定的缓解作用,提示交感神经参与神经病理性痛自发痛的产生与维持。该文从与交感神经影响神经病理性痛自发痛相关的外周及中枢神经系统可塑性改变、介质基础等方面做相应综述,以期为临床更有针对性地治疗神经病理性痛自发痛提供思路。
Effect of Sympathetic Nerve on Neuropathic Spontaneous Pain
Spontaneous pain is continuous or paroxysmal pain that is not related to an external stimulus.It is one of the main clinical manifestations of neuropathic pain.Due to its complex pathogenesis and lack of effective treatment measures,spon-taneous pain brings great trouble to patients with neuropathic pain.It has been revealed that neuropathic spontaneous pain is re-lated to dysfunction of sympathetic nervous system.In clinical medicine,neuropathic spontaneous pain could be depressed by sympathetic nerve block or sympathectomy,suggesting that sympathetic nerve is involved in the generation and maintenance of neuropathic spontaneous pain.In this review,peripheral and central nervous system plasticity changes and mediators basis relat-ed to sympathetic nerve mediating neuropathic spontaneous pain will be reviewed.Hopefully it might provide more ideas for clinical treatment of neuropathic spontaneous pain.
