Plasma concentration monitoring and analysis of influencing factors after risperidone administration in patients with mental illness
Objective:To investigate the monitoring results of plasma concentration of risperidone in patients with mental illness and its influencing factors. Methods:The medical records and risperidone plasma concentration test results of 60 psychiatric patients admitted to the Third People's Hospital between January 2022 and December 2023 were retrospectively analyzed, and the differences in risperidone plasma concentration among patients with different baseline data were compared. The relationship between risperidone concentration/dose ratio(C/D),risperidone active metabolite(9-hydroxyrisperidone)and parent drug(risperidone)concentration ratio(MPR),and patient age was analyzed, and the risk factors affecting risperidone concentration were analyzed using multiple linear regression. Results:Among the 60 patients,12(20.00%)had plasma concentrations of risperidone that were not within the reference range, and the average plasma concentration was(46.40±14.33)ng/ml.The average MPR was 5.59(2.12 for P25 and 10.55 for P75).The average C/D value was 9.91(7.66 for P25 and 14.12 for P75).There were statistical differences in dose and plasma concentration among different age groups(P<0.05),but there were no statistical differences in MPR and C/D values among different age groups(P>0.05).The dosage of medication in males was higher than that in females, and the C/D value was lower than that in females, with statistical differences(P<0.05).However,there was no statistically significant difference in the MPR and plasma drug concentration between the sexes(P>0.05).The MPR of the<18.5 kg/m2 group was higher than that of other weight groups(P<0.05),but there was no statistically significant difference in body mass index(BMI)dosage,C/D value,or blood drug concentration(P>0.05).Multiple linear regression analysis showed that gender, dosage, and BMI were high-risk factors affecting the plasma concentration of risperidone(P<0.05).Conclusion:There is a significant individualized difference in plasma drug concentration among psychiatric patients after oral administration of risperidone, which is influenced by gender, dosage, and BMI.
