穿刺垫棉绑缚法治疗浆细胞性乳腺炎脓肿的临床研究

[目的]评价穿刺垫棉绑缚法治疗浆细胞性乳腺炎脓肿的临床疗效及安全性.[方法]应用随机对照临床试验方法,共纳入患者 72 例,分为穿刺垫棉绑缚组和切开常规换药组两组,按 2∶1 进行分配,穿刺垫棉绑缚组纳入48 例,采用穿刺抽脓后垫棉绑缚的方法治疗,切开常规换药组纳入 24 例,切开引流后根据疮口情况换药治疗,疗程均为8周,分别观察两组患者脓肿缩小程度、疼痛持续时间、视觉模拟评分(VAS)、乳房外形改善、换药次数、不良反应.[结果]穿刺垫棉绑缚组在脓肿缩小程度、乳房疼痛时间、VAS评分,乳房外形改善、换药次数方面与切开常规换药组相比,差异有统计学意义(P<0.05),未见明显不良反应.[结论]穿刺抽脓、垫棉绑缚法治疗浆细胞性乳腺炎脓肿临床有效,值得推广应用.
Clinical study on treatment of plasma cell mastitis in abscess stage with pad and binding after puncture
[Objective]To evaluate the clinical efficacy and safety of puncture pad cotton binding method in the treatment of plasma cell mastitis abscess.[Methods]A prospective randomized controlled clinical trial was conducted,with a total of 72 patients included.They were divided into two groups:the puncture pad cotton binding group and the incision routine dressing change group,with a ratio of 2∶1.The puncture pad cotton binding group included 48 patients,who were treated with puncture aspiration followed by pad cotton binding.The incision routine dressing change group included 24 patients,and after incision and drainage,dressing change treatment was performed according to the condition of the wound.The course of treatment was 8 weeks,and the degree of abscess reduction in both groups was observed.Duration of pain,VAS pain score,improvement in breast appearance,frequency of dressing changes,and adverse reactions were observed too.[Results]There were significant differences(P<0.05)in the degree of abscess reduction,breast pain time,VAS score,improvement in breast appearance,and frequency of dressing changes between the puncture pad cotton binding group and the conventional dressing changing group,and no significant adverse reactions were observed.[Conclusion]The puncture aspiration and cotton pad binding method is clinically effective in the treatment of plasma cell mastitis abscess and is worth promoting and applying.
