Analysis of current status and influencing factors of readmission of children in neonatal intensive care unit
Objective To understand the current status of readmission of neonatal intensive care unit(NICU)children and the related influencing factors,to provide a reference for the later clinical practice of critically ill neonates.Methods To retrospectively collected the clinical data of children admitted to the NICU in a Class Ⅲ Grade A hospital in Fujian Province from August 2020 to July 2022,and based on the inclusion and exclusion conditions,224 children who were readmitted were included in the readmission group.,and then used a 1:1 case-control study method to select 224 children who had not been readmitted as the non-readmitted group.Next,we collected the following data of the two groups of children,including admission diagnosis,gender,gestational age at birth,birth weight,birth length,Apgar score 1 min after birth,whether to use a ventilator,C-reactive protein(CRP)at discharge,white blood cells,data on blood bilirubin levels,mother's mode of delivery,mother's age,maternal parity,etc.Then analyzed the influencing factors of readmission of NICU children by using univariate analysis and binary Logistic regression.Results The readmission rate of NICU children was 1.75%(224/12816);150 cases(66.96%)were readmitted within 1 week of discharge,and 43 cases(19.20%)were readmitted 1 to 2 weeks after discharge;the main diagnosis were neonatal hyperbilirubinemia in 101 cases(45.09%),digestive system diseases in 35 cases(15.63%),respiratory system diseases in 34 cases(15.18%),and perinafal infection in 33 cases(14.73%).Logistic stepwise regression analysis showed that the gestational age and CRP level at birth are the factors affecting the readmission of children in NICU.Conclusion Gestational age and CRP level at birth are the main factors affecting the readmission of children in NICU.NICU medical staff should comprehensively consider the gestational age of children and CRP level at discharge,promptly solve the difficulties of parents in the care process,and remind parents follow up regularly and try to ensure that the CRP level is within the normal range when discharged,in order to reduce the readmission rate of NICU children.
